0286865853 2019-01-15 Final Restoration No Violations The purpose of my inspection was to document the final restoration status of well #6-0985. The well location was at the boundary of an agricultural field. At the time of my inspection the location was partially covered in snow. The pit was backfilled. No equipment remained. No buried flow lines existed. No debris was present. No production equipment was on location. The location was stabilized with vegetation. I did not witness any signs of erosion. No access road was present. The location meets the requirements for final restoration.
0459655983 2017-01-23 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #6-0985. A review of RBDMS indicates the well owner has not reported production in the last two years. The well location was in an agricultural field and was stable. There was no active access road. I found the well to be idle. I did not observe any identification. The well head was above grade and equipped with a stuffing box style unit. A pumping rod extended from the top of the stuffing box. No pumping unit was present on the well head. No audible or visual leaks were identified. I did not observe any production equipment. I will contact Bob Foltz concerning plans to produce the well.
1530540366 2018-07-02 Plug / Plug Back No Violations The purpose of my inspection was to witness plug and abandonment activities on well #6-0985. At the time of inspection all lift equipment was removed from the well and owner Bob Foltz was attempting to bail the well dry. Mr. Foltz reported the fluid level at 430 feet below surface. Total depth was measured at 744 feet below surface. Mr. Foltz was not able to bail the well dry and installed 2 inch tubing to a depth of 715 below surface to act as a tremie line. Prior to pouring grout into the well, the tubing was flushed to ensure no obstructions were present to impede flow. R.W. Sidley delivered and poured 1.75 yards of 9 sack grout into the well bore. No flow was observed from the tremie line. Mr. Foltz attempted to pull the 6.25 surface casing from the well. The rig pulled a total of 50,000 pounds against the casing without any movement. Mr. Foltz pulled a second time and placed an additional 17,000 pounds against the casing with the assistance of an track excavator. On the second attempt the top joint of casing tore free. I inspected the threads and witnessed that approximately 1/2 of contact had been present between the connections. The others threads were rusted away and the connection could not be made up again. Operations concluded for the day. I will schedule to witness the surface plug. The earthen lined pit continued to hold fluid and maintained freeboard.
1531156124 2018-07-03 Plug / Plug Back No Violations The purpose of my inspection was to witness plug and abandonment activities on well #6-0985. At the time of inspection owner Bob Foltz tagged the top of the first cement plug at 453 below surface. Mr. Foltz bailed the free fluid from the well and prepared to set the surface plug. R.W. Sidley delivered and poured 3.25 yards of 9 sack grout into the well. The top of grout was not brought to surface. Mr. Foltz planned to top the surface plug with bag mix cement prior to cutting the casing below grade and backfill. I will schedule a follow-up inspection to document the status of the location.
1673713904 2015-11-18 Production Wells No Violations My inspection found the well to be equipped with 6 1/4 casing, 2 tubing and rods. The pumpjack has been removed from the well. The annulus is open to the atmosphere. There is no tank battery associated with the well. There is no known production ever recorded for the well. The well owner is currently working to plug other berea wells that they own.
