The side gate valve is plumbed into a 4.00 lead line that runs into the main line. The well is properly identified with a clear, legibly marked well identification sign. The lease road is in stable condition and drivable. No violations found.
0450047687 2014-11-10 Storage Well No Violations I inspected Columbia Gas Transmission gas well #3641. The well is equipped with a master gate valve that is plumbed up to a side gate valve. The side gate valve is plumbed into a 4.00 lead line that goes into the main line. The annulus valve is plumbed up and closed. There was a sales line with a separator. The well is properly identified with a clear, legibly marked well identification sign. The lease road is in stable condition and drivable. No violations found.
1609190733 2017-11-06 Storage Well No Violations The purpose of my inspection was to conduct a status check on well #6-0267. Correct identification was present. Annular side ports were plugged. The production casing was equipped with a production cross that was topped with a capped master gate valve. One side of the cross contained a capped side gate valve. The other side of the cross contained a side gate valve that was connected to a 4.5 run that lead to the main line. A buried drip was also present. No leaks were observed. No violations were observed during this inspection.

2028036739 2018-08-15 Storage Well No Violations The purpose of my inspection was to conduct a status check on well #6-0267. Access to the well remained drivable and stable. Correct identification was present. The surface casing well head was above grade and closed to atmosphere. The well head was fitted with a master gate valve topped with a flow cross. The cross was topped with a snubbing connection. The South side of the cross was capped. The North side of the cross contained a wing valve that was fitted to a 4.5 run that lead to a buried drip. All valves were closed. No leaks were observed.
