On 2/14/2017 I received an E-mail from Stephanie McMurray with Hilcorp Energy stating that hydraulic fracturing was to occur beginning the week of 2/20/2017. At the time of my inspection the drilling rig and related equipment were off location. A steel grate was placed over the cellar to prevent falls. The cellar contained a minimal amount of water. I did not observe any audible or visual leaks from the well head. I met with Hilcorp Energy Senior Wellsite Supervisor Kenneth Shimek to discuss plans for the pad. Mr. Shimek stated that a coil tubing unit was scheduled to make a clean out run on the production casing followed by pressure tests prior to stimulation. Stimulation would be occurring on or before 3/1/2017. I communicated to Mr. Shimek the requirement to notify an inspector at least 24 hours prior to commencing stimulation. I will plan to witness the stimulation work once it commences. |

0573718604 | 2016-12-10 | Casing String Witnessed | No Violations | I was on location at 12:45am with Columbiana County Inspector Ken Frye to witness the cementing of the 9.625 intermediate casing string for this horizontal shale well. H&P Flex Rig #225 drilled a 12.25 hole to a depth of 1659 (KB +26). 40 joints of 36lb/ft., J-55 casing was set at a depth of 1646 (KB) and run with a float shoe on bottom, a float collar at 1563 (KB) and equipped with 14 bow-spring centralizers. The casing crew had trouble landing the fluted mandrel and the landing joint was swapped out for longer one and they landed the mandrel in the casing hanger. Baker-Hughes (BH) rigged up the cement head with the bottom plug and the safety meeting was conducted at 5:00am. BH then loaded the lines, pressure tested the lines to 3000psi, and dropped the bottom plug. BH then pumped 20 barrels of gel, 10 barrels of freshwater spacer, and then mixed and pumped 126 barrels (595 sacks) of Class A cement slurry. The top plug was released from the cement head and H&P Rig #225 took over to displace the plug with oil based mud. Columbiana County Inspector Frye witnessed the remainder of the job. See casing report for more details on this job. No violations. |

I met with Hilcorp location foreman Jerry Myers on location. TJD Energy Service was on location completing pipelining and production operations. Correct and legible identification was present on the well head. Steel grate was in place over the cellar to prevent falls. A sand knock out vessel was plumbed out from the well head. No audible or visual leaks were observed. Production equipment on location consisted of three (3) heated separators, one (1) glycol dehydrator, two (2) 400 bbl produced water tanks, one (1) combustion stack, and one (1) flow back flare stack. The combustion stack was flaring at the time of inspection. I walked the perimeter of the location and did not observe any issues of erosion or sedimentation. Pad slopes were stable. Vegetation was becoming established. No violations were observed during this inspection. |

The complainant, Mr. Bryan Wisler stated that he was hearing loud pressure release noises coming from the pad. While on location I did not experience any sounds of excessive pressure release. I did observe that a gas dehydration unit was removed from location. I called and spoke with Hilcorp Energy wellsite supervisor Kenneth Shimek regarding the complaint and equipment. Mr. Shimek stated that the unit had been removed and that the production system was blew down and sent to the vapor recovery flare. He continued any other releases were due to choke changes on the well to optimize production. While on location I did not experience any audible pressure releases. I will contact Mr. Wisler concerning the source and cause of the release. |

H&P Flex Rig #225 drilled a 17.5 hole to a depth of 828 (KB +26). 19 joints of 54.5lb/ft., J-55 casing was set at a depth of 826 (KB) and run with a float shoe on bottom, a float collar at 778 (KB) and equipped with 8 bow-spring centralizers. The rig circulated the hole for one (1) hour after landing the casing 6 above the rig floor. Baker-Hughes rigged up their iron and the cement head and pressure tested the lines to 3000psi. Baker then pumped 50 barrels of freshwater spacer and then mixed and pumped 153 barrels (715 sacks) of Class A cement slurry. The plug was released from the cement head and the plug was displaced to the float collar with 123 barrels of water. The plug landed at 420psi and the pressure was increased and held at 920psi for 7 minutes with no leak-off. The pressure was released, the floats held, and 0.5 barrels of water flowed back to the truck. See casing report for more details on this job. No violations. |

The complainant stated that drilling activity had begun on the Fairfield-Unkefer Unit and that her water had not been sampled. Further, the complainant stated her lease required her water supply to be sampled in the event drilling activity were to take place. I reviewed the map supplied with the drilling permit application and found the complainants parcel to be outside of the 1500 feet perimeter of sampling required by 1509(A)(8)(c). The complainants parcel was also found to be outside of a 3000 feet perimeter drawn on the same map. I independently measured the distance from the nearest permitted well location to the complainants house and found that distance to be over 3500 feet. I called and spoke with the complainant at 08:44 on 12/09/2016. I explained to the complainant that the reason her water supply was not sampled prior to drilling was due to the location of her property outside of the perimeter that required sampling. I inquired if she had experienced any problems with her water supply to date. She replied that the only issue to date had been low pressure. I communicated to the complainant that if she experienced any problems in the future to please contact us again. I will update the Complaint Log with this information. No further action is necessary at this time. |

I received a phone call from Hilcorp Energy Representative Ken Shimek stating that the Company had production concerns regarding the well and planned to divert the well to flare around 10 AM, and would return the well into the sales line at some time later that evening. Following the call with Mr. Shimek, I called Northeast Region Program Administrator Wendy Booth to report the plan to flare. I arrived on location at 13:45 to document conditions on the pad. I met with Hilcorp Energy Field Foreman Jerry Myers on location. Mr. Myers stated the well was bringing sand to surface and causing issues with surface production equipment. I witnessed surface equipment undergoing maintenance while the well was flowed through flow back equipment operated by Greenes Energy Group and flared through the flow back flare stack and production flare stack. I did not observe any spills or leaks of production fluid on the pad. Surface contractors were also reclaiming the top soil stock pile at the time of inspection. Erosion and sediment controls remained in place and functioning. No violations were observed during this inspection. |

On 11/09/2016 I received a phone call from Hilcorp Energy Company (HEC) contracted drilling consultant Bill Mitchell giving notice that HEC planned to prepare for the drilling and setting of conductor casing on 11/10/2016. I communicated to Mr. Mitchell that ODNR-DOGRM was requesting a pre-spud meeting on location to review special permit conditions and establish a line of communication between HEC representatives and ODNR-DOGRM Inspectors during drilling operations. On 11/10/2016, Mr. Mitchell, ODNR-DOGRM Inspectors Michael Dinsmore and Kenneth Frye met to review permit conditions and discuss the processes and procedures that will occur during the active drilling phase on the pad. A review of the Owners proposed casing plan indicated the casing seat depths on the conductor, surface and intermediate casing strings did not correspond with the permitted casing program. After discussion, Mr. Mitchell stated the Owner would revise the proposed casing program to follow the permitted casing program for conductor and surface casing. The intermediate casing seat revision proposed by the operator was deemed acceptable based on intermediate casing seat depths on the Owners previous wells in the nearby area; under the condition that the seating depth would not impair well control efforts. I will plan on being on location to witness the setting of conductor casing. |
