-based mud used to drill and condition the hole was pumped at 13.5 pounds per gallon (ppg). Lastly, the tail cement was pumped. The cement slurry was mudscaled to verify the target density. Nabors pumped 536 bbl of tail cement at 14.4 ppg. The plug was dropped and displaced with 294 bbl of water. The plug bumped the float collar at 1:39pm on 03/06/2015. The plug landing pressure was 2800 psi, and it was raised to 3410 psi, and held for 5 minutes. Pressure held. The float held when the pressure was released. 3.5 bbls of water bled back.

1954811578 2016-08-23 Production Wells No Violations The purpose of my inspection was to conduct a status check of the Firestone Homestead Pad A with Regional Emergency Operations Manager Terry Whitehair, and Emergency Operations Coordinator Fred Romeo. Atlas Lease Operator Nelson Case provided entry to the pad and served as a guide for the inspection. The pad location is within an urbanized area. Access to the pad is over a lease road that is lined with crushed gravel and remains in good repair. A locking entrance gate constructed of tubular steel is present. A sign board posted near the entrance identified the operator, well names and permit numbers, and emergency contact information. I walked the perimeter of the pad and found the exterior perimeter stable with well- established vegetation. The topsoil stockpile is stabilized with well-established vegetation. The surface of the pad was composed of crushed gravel that remained in good repair. The Firestone Homestead Pad A currently contains four (4) wells. Firestone Homestead Pad A #1H (API # 2-1853-00-00) is identified by a sign posted on the chain link security fence topped with barbed wire. After stimulation, the well was equipped with a Christmas tree valve assembly. The casing pressure gauge indicated 1000# and the tubing pressure gauge indicated 850#. The intermediate casing gauge indicated 0 psi and was plumbed to the GPU brine tank. The cellar had plastic pipe installed to facilitate draining and was backfilled with gravel. The mousehole was cemented. No audible or visual leaks were detected. Production equipment on the pad consisted of four (4) gas production units (GPU), having one unit for each respective well, one (1) 210 bbl GPU brine tank and three (3) 360 bbl production tanks. The GPU contained correct, legible and conspicuous identification and sat adjacent to the units for the other three wells on the pad. All four units were placed on top of containment that remained in good repair and contained no free fluid. No audible or
