0066273468 2016-12-06 Production Wells No Violations The purpose of my inspection was to conduct a status check on well # 2--established vegetation. A sign board is posted at the entrance and contains correct owner and well identification and emergency response information. A chain link security fence with barbed wire top is constructed around the well pad. The entrance gate and escape doors are locked to prevent unauthorized entry. An electric shut down button is present outside of the entrance gate. An earthen berm perimeter with well-established vegetation is present around the fence/ well pad. The well pad surface is composed of crushed gravel and remains in good repair.

Correct and legible identification is posted at the well head. The well cellar is backfilled with gravel and contains a pipe to remove water. The mousehole is cemented. The Owner has raised the annular casing pressure gauges on the intermediate and production casing gauges. Both gauges indicated 0 psi. Tubing has been placed inside the 5.5 production casing. The digital production casing pressure gauge indicated 312 psi. The digital production tubing pressure gauge indicated 163.8 psi. No audible or visual leaks were detected. Production equipment located on the pad consists of two (2) vertical sand/fluid separators, two (2) gas processing units(GPU), one (1) 350 gallon hydrate inhibitor tank within sealed containment, one (1) thermal electric generator, six (6) 400 bbl production tanks within containment and one (1) emission combustion tower. Concrete jersey barriers are placed in front of the production equipment to prevent damage. The vertical separator and GPU specific to well # 2-1844 are in good repair with no audible or visual leaks. Well specific identification is posted on the GPU. No audible or visual lea

0667847011 2016-03-02 Preliminary Restoration No Violations I was on location for a status check. My inspection found the access road gate locked and the access road is in good repair. An identification sign at the access road entrance, well, and tank battery. The pad is fenced and all man doors are locked. The #1H well is in production. The well is equipped with a christmas tree valve assembly. There is a steel dike around the tank battery.

The equipment on the pad consists of the following: 6 - Storage tanks 400 bbl 1 - Vapor recovery unit (VRU) 2 - Gas processing units (GPU) 1 - Vertical separator 2 - Sand separators 1 - Thermal electric generator 1 - Heat treater 1 - Compressor 2 - Producing wells Site fenced and man doors locked

1397480423 2014-04-12 Drill / Deepen / Reopen No Violations I was on location for the cementing of the conductor casing. The permit number and the emergency information were posted at the entry of the lease road. The lease road was stable and drainage controls were in place. Butchs Rat Hole & Anchor Service drilled a 30 diameter hole to 100. Butchs installed 100 of 94lb, 20 OD casing. O-Tex was the cementing contractor. Circulation was established. The cement was mudscaled at 15.6 pounds per gallon (ppg). O-Tex pumped 78 bbls of cement slurry at 15.6 ppg. The cement was displaced with 32 bbls of water. Cement returns to surface were observed. An estimated 15 bbls of cement returned to surface. No violations found.
1399291316 2014-05-03 Casing String Witnessed No Violations I went to the site to witness the cementing of the surface casing. The drilling contractor, Nomac 43, spudded the well on 5/3/2014. The 17.50 diameter surface hole was drilled to 605. The well made water at approximatley 440. Once the well stopped producing water, circulation established after 100 BBLs of fresh water was pumped down hole. The 13.375 OD surface casing was set at 597. A total of five centralizers were installed on the surface casing string. O-Tex was the cementing contractor. The cement was mudscaled at 15.6 lbs per gallon (ppg). O-Tex pumped 111 bbls of cement slurry at 15.6 ppg. Cement returns to surface were observed. 20 bbls of cement returned to surface. The plug was displaced with 85.8 bbls of water. The plug bumped the float collar and O-Tex pressured up on the casing, and the pressure was held for 5 minutes.
