-based mud used to drill and condition the hole, a lead cement followed by a tail cement were pumped. The cement slurries were mudscaled to verify the target density. Schlumberger pumped 303 bbls of lead cement at 13.5 pounds per gallon (ppg), and 385 bbls of tail cement at 15.0 ppg. The plug was dropped and displaced with 335 bbls of water. The plug bumped the float collar at 4:48am on 10/04/2013. The plug landing pressure was 2600 psi, and it was raised to 3330 psi, and held for 5 minutes. The float held when the pressure was released, and 4.0 bbls of water bled back. Approximately 60 bbls of the Mudpush spacer returned to surface.

0966991336 2013-09-17 Drill / Deepen / Reopen No Violations I went to the site to witness the cementing of the surface casing. The drilling contractor, Nomac 77, drilled a 17.50 diameter surface hole to 645. There were no issues while drilling the surface hole. Circulation was maintained throughout the drilling. The 13.375 OD surface casing was set at 635. A total of seven centralizers were installed on the surface casing string. Schlumberger was the cementing contractor. There was good fluid circulation throughout the cementing operation. The cement was mudscaled at 15.6 lbs per gallon (ppg). Schlumberger pumped 123 bbls of cement slurry at 15.6 ppg. Cement returns to surface were observed. Returns mudscaled at 15 ppg. An estimated 34 bbls of cement returned to surface. The plug was displaced with 92 bbls of water. The plug bumped the float collar and was pressured up to 500 psi and held for 5 minutes. The plug did hold. The permit was in the company mans trailer. There were fire extinguishers next to every Schlumberger truck. No violations.
1029284104 2017-02-28 Production Wells No Violations The purpose of my inspection was to document the repair of a gas leak observed from the 5.5 production casing annulus during my previous inspection. At the time of re-inspection, the Owner had pulled the cellar water level down to approximately 24 of free board. Per E-mail correspondence with Chesapeake Regulatory Manager Jody Jones, the leak was identified and repaired the day after my initial inspection on 2/9/2017. Mr. Jones indicated the leak source was a connection between a nipple and elbow. Annular pressure gauges on the 9.625 and 5.5 casing both indicated 0 psi. The 5.5 production casing gauge indicated 272 psi. No further action is necessary at this time.

-lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Along the interior of the chain link fence there is a site drain installed. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete and approved.

There is welders and roustabouts on location installing equipment. I did not walk the entire site due to the active. All erosion and sedimentation controls, and drainage controls appear to be functioning. Ground cover is well established along the access road and around the drilling pad. No areas of erosion are present. Preliminary restoration approval will not be finalized pending installation of production equipment.

1666187757 2017-02-09 Production Wells No Violations The purpose for my inspection was to conduct a status check on well #2-1818-00-00). RBDMS indicates the operator is current with reporting production. The lease entrance is marked with a sign board containing signs reporting required well identification and emergency response information. At the time of my inspection, the local area had received a coating of snow that covered the location. The lease road was drivable. No signs of erosion were observed adjacent to the road traveling back to the pad. The soil stockpile remains stable with well-

The area of disturbance along the access road and the pad, along with the soil stockpile, have been seeded and stabilized. Drainage and erosion controls were in place. Both the access road and pad have a limestone gravel surface. Neither a fence nor gate had been installed. No wells had yet been staked.
