0052495460 | 2018-03-05 | Production Wells | No Violations | The purpose of my inspection was to conduct a temporary inactive status permit renewal inspection on well #2-1789. The lease road and pad surface was composed of crushed gravel and remained in good repair. Correct identification was posted at the entrance and well head. A locked chain link fence remained around the pad. The perimeter earthen berm remained stable with well established vegetation. No signs of mass wasting were observed. The well head was protected by jersey barriers. The cellar contained freshwater with no sheen. Steel grate covered the cellar to prevent falls. The annular gauges were plumbed above grade and identified. The 9.625 and 5.5 annular valves were open. The 9.625 gauge indicated 65 psi. The 5.5 annulus gauge identified 380 psi. The 5.5 production casing was equipped with a Christmas tree valve assembly. The production casing gauge indicated 0 psi. Gate valves were closed, chained and locked. No leaks were observed. The operator reported in the application that the well was completed and is waiting on pipeline. No violations were observed during this inspection. |
0125117030 | 2016-03-31 | Production Wells | No Violations | I was on location for a status check. My inspection found the access road gate locked and the
access road in good repair. The pad is fenced and all man doors were locked. An identification sign is posted at the access road entrance and well. The #10H is not in production. The well is equipped with a christmas tree valve assembly. The equipment on the pad consists of the following: Site fenced and man doors locked |

-based mud used to drill and condition the hole, a lead cement slurry followed by a tail cement slurry were pumped. The lead cement was mudscaled at 13.5 pounds per gallon (ppg). Schlumberger pumped 300 bbls of lead cement at 13.5 ppg. Tail cement was mudscaled at 14.4 ppg, and adjusted to 15.0 ppg. Schlumberger then pumped 376 bbls of tail cement at 15.0 ppg. The plug was dropped and displaced with 322 bbls of water as measured by tanks. The plug bumped the float collar at 1:52am on 04/15/2013. Pressure held. The float held when the pressure was released, and 3.5 bbls of water bled back. All of the spacer returned to surface, and an estimated 5 bbls of cement. | |

1467996304 | 2016-06-23 | Production Wells | No Violations | My purpose for inspection was to conduct a first year temporary inactive well status inspection.
There is one well on the pad. The sign board posted at the lease entrance displayed signs with the well name, address, permit number and emergency contact information. The lease road was graveled and in good repair with a gate at McSwiggen Road. Erosion and sedimentation controls were in place and ground cover was well established along the lease road. I walked the entire location and found the well pad constructed with an earthen berm perimeter in good repair, being stabilized with well-established vegetation. Erosion and sedimentation controls are in place and functioning. The well pad was found to be enclosed by a chain link fence with barbed wire at the top. The primary entrance and escape doors were found locked. The perimeter of the earthen berm is further bounded by an electrified cattle fence. The surface of the well pad consists of gravel and is in good repair. A perimeter drain is installed and both stand pipes were shut. The pad contained areas of standing water. The Manziel 10H was constructed with 120 of 20, 831 of 13 3/8 surface casing, 1552 of 9 5/8 intermediate casing and 14525 of 5 1/2 production casing. Drilling was completed 4/13/2013 and stimulated 5/6/2013. The well head was constructed with a Christmas tree valve assembly and found to be protected by a perimeter of steel cattle gates further protected by concrete Jersey barriers. Proper signage was displayed. A cellar grate was in place to prevent falls. The cellar contained water with approximately 1 foot of freeboard. A slight geenish blue sheen was observed on the cellar water surface. No leaking gas was observed on the water in the cellar. An annular pressure gauge indicated 400 psi. The production gauge indicated 0 psi. No other gauges were observed. The mousehole was cemented. Gate valves are chained and locked. No other production equipment is on the pad. I |
1487188119 | 2017-08-29 | Production Wells | No Violations | The purpose of my inspection was to conduct a status check on well #2-1789. The well had a valid temporary inactive status permit. The lease road and pad surface was composed of crushed gravel and remained in good repair. Correct identification was posted at the entrance and well head. A locked chain link fence remained around the pad. The well head was protected by jersey barriers. The cellar contained freshwater with no sheen. Steel grate covered the cellar to prevent falls. The annular gauges were plumbed above grade and identified. The 9.625 and 5.5 annular valves were open. The 9.625 gauge indicated 50psi. The 5.5 annulus gauge identified 450 psi. The 5.5 production casing was equipped with a Christmas tree valve assembly. The production casing gauge indicated 0 psi. Gate valves were chained and locked. No leaks were observed. No violations were observed during this inspection. |
1543420984 | 2018-11-26 | Production Wells | No Violations | The purpose of my inspection was to conduct a status check on well #2-1789. Correct identification was present at the entrance. The road and well pad remained in good repair. The perimeter security fence remained in good repair and locked. The earthen berm perimeter remained stable. Correct identification was present at the well head. Jersey barriers were present to prevent a collision. Grates remained over the cellar to prevent falls. The cellar contained freshwater. No leaks were observed. Gauges to measure annular casing pressure were present and labeled. The 9.625 annulus indicated 60 psi. The 5.5 annulus indicated 375 psi. The 5.5 production casing wing valve gauge was not present. The production tree cap gauge indicated 0 psi. No production equipment was on location. No violations were observed during this inspection. |

-established ground cover. The soil stockpile is well stabilized with vegetation. Crops, woodlands and/or underbrush are growing normally outside the area of disturbance. Drainage and erosion controls outside the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure.
