1175382423 2016-11-09 Production Wells No Violations The purpose for my inspection was to conduct a status check on well # 2-1763-01. Entrance to the well pad is marked by a sign board that contains correct and legible identification for the well on the pad along with required emergency response information. Lease access is over a crushed gravel roadway that remains in good repair. A locked gate is in place to prohibit unauthorized access. Erosion and sedimentation controls along the lease road remain in place and are functioning. The topsoil stockpile is stabilized with well--established vegetation. Correct and legible identification is not present at the well head. The Owner has placed concrete Jersey barriers around the perimeter of the well head to prevent accidental damage. The cellar has been backfilled with crushed gravel and contains a pipe to facilitate draining. The mousehole was cemented. Well # 2-1763-01 is completed with tubing installed inside the 5.5 production casing. Digital gauges indicated 2622 psi on the casing and 274 psi on the tubing. A soap injection unit was also present to facilitate lifting production fluid. No audible or visual leaks were detected. Production equipment on the pad consisted of one (1) gas processing unit (GPU) with sand dump, one (1) glycol dehydrator, one (1) dry dessicant tower and two (2) 360 bbl production tanks and one (1) open burn flare stack. The GPU identified for well # 2-1763-01 remained in good repair and showed no evidence of audible or visual leaks. A sand dump was plumbed in line with the GPU and discharged to an open top tank. The tank contained minimal fluid. The drain valve on the tank did not contain a bull plug. A dry chemical fire extinguisher was present and last inspected in November 2016. The glycol dehydrator was not in operation. A dry

Correct and legible identification is posted at the well head. A bull plug is installed in the drain valve on the open top box. The tank contained production fluid from the gas processing unit. Freeboard level on the tank was approximately 36. No further action is necessary.

We found a new gate with a padlock at the access road entry. The pad was in good shape. The cellar had plywood sheets for cover. A grate is needed. I will contact the company regarding replacing the sheets with a grate. The well has not been hydraulically fractured. There is no equipment on site, but a single frac tank.

All equipment on the drilling pad has been removed except for one frac tank. The secondary containment was still on the pad. 1734830089 2013-10-24 Preliminary Restoration No Violations I reviewed the site to assess the completion of preliminary restoration. There is a sign board at the access road entry featuring the well name, and emergency contact information. There is also a copy of the drilling permit posted. The access road has a padlocked gate near Grafton Road. The access road was in good repair, and drainage and erosion controls were in place. Ground cover was well established along the access road. The pad has a soil--lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. The tanks are grounded. There is a truck grounding area for trucks collecting oil and/or produced water. There is also additional production equipment, and all of it is protected from vehicle collision with concrete barriers. There is no site drainage system installed. The soil stockpile is well stabilized with ground cover. The area of disturbance surrounding the pad has good ground cover. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. I will contact the company regarding plugging the mouse hole. Preliminary restoration is complete.
