-based mud used to drill the hole and condition the hole, two stages of cement slurry were pumped. The lead cement was mudscaled at 14.4, then 14.3 pounds per gallon (ppg), and the density adjusted. Halliburton pumped 181 bbls of lead cement at 14.2 ppg. The tail cement was mudscaled at 15.6 ppg. Halliburton pumped 261 bbls of tail cement at 15.6 ppg. The plug was dropped, and displaced with 304 bbls of water. The plug bumped the float collar at 1:07am on 12/18/2012. Pressure held. The float held when the pressure was released. 4.0 bbls of water bled back.

0924008942 2014-06-17 Preliminary Restoration No Violations I stopped by the site to check on the status of well completion and restoration. The well has been hydraulically fractured. There is two 400 barrel tanks and one GPU production unit. The well is equipped with a Christmas tree. Values are locked out. Tanks or the wellhead do note have ID signs posted. I walked the entire site, and found the location to be in good condition. All erosion and sedimentation controls, and drainage controls appear to be functioning. Ground cover is well established along the access road and around the drilling pad. No areas of erosion are present. Preliminary restoration approval will not be finalized until well is in production and ID signs are posted.

-lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. There is an earthen berm, with well-established ground cover. The soil stockpile is well stabilized with vegetation. Crops, woodlands and/or underbrush are growing normally outside the area of disturbance. Drainage and erosion controls outside the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete and has passed inspection.

1450940067 2017-05-18 Production Wells No Violations The purpose of my inspection was to respond to complaint #5613. I received a phone call from well pad surface owner Lisa Mueller. The complainant stated that she was periodically hearing humming noises that were increasing in intensity and frequency. She continued that on the evening of 5/17/2017, the noise increased to a level that was causing her residence to vibrate. The ODNR-DOGRM Oil and Gas Well Locator indicates the residence is approximately 1400 feet to the Northeast of the well pad. While at the residence interviewing the complainant I did not witness any noises as described. I requested that she document any future times and durations of observed noises or vibrations. I also communicated to her that the noises that were occurring were likely due to surface equipment in need of adjustment, and that I had placed a call to Chesapeake Energy Regulatory Affairs Manager Jody Jones requesting that a lease operator visit the location as soon as possible to make any adjustments or repairs. I will follow up with Mr. Jones to confirm action was taken to reduce or eliminate future disruptions. I will also communicate activity progress to the complainant.

The lease entrance is marked with a sign board containing signs reporting required well identification and emergency response information. The lease road exhibited signs of erosion but remained drivable. No signs of erosion were observed adjacent to the road traveling back to the pad. The soil stockpile remained stable with well-

An identification sign at the access road entrance, well, and tank battery. The pad is fenced and all man doors are locked. The #3H well is in production. The well is equipped with a plunger lift assembly. There is a steel dike around the tank battery.
