-14--lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting produced water. There is also additional production equipment, and all of it is protected from vehicle collision with concrete barriers. There is no site drainage system installed. Outside the fence, there is an earthen berm with fairly good ground cover. Some areas of the berm are shaley with sparse growth. The soil stockpile is well stabilized with ground cover. The area of disturbance surrounding the pad has good ground cover. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete.

0384829900 2012-10-24 Drill / Deepen / Reopen No Violations I was at the site checking the progress of the drilling of the conductor hole for the conductor casing. Express Energy Services was on site drilling the conductor hole. Express was also drilling the conductor for the Milltree 3H. Express expected to finish the holes today, and install and cement the conductor casing the next day (10/25/2012).
0498474154 2017-01-24 Production Wells No Violations The purpose for my inspection was to conduct a status check on well #2-1754-00-00. RBDMS indicates the operator is current with reporting production. The lease entrance is marked with a sign board containing signs reporting required well identification and emergency response information. The lease road is also utilized by the surface owner and was stable and drivable. No signs of erosion were observed adjacent to the road traveling to the pad. An earthen dike perimeter is erected around the well pad. A chain link fence with barbed wire top is constructed around the pad to prevent unauthorized entry. An Emergency Shut Down (ESD) button was adjacent to the main entrance gate. The main entry gate and escape doors positioned around the perimeter were found to be closed and locked. The pad surface was composed of crushed gravel and remained in good repair. I observed water ponding along the outer edge of the pad adjacent to the berm. The soil stockpile was stabilized with well-

Drainage and erosion controls at the pad are in place and functioning. No Violations observed.

0671023325 2012-10-25 Drill / Deepen / Reopen No Violations I witnessed the cementing of the conductor casing. Express Energy Services drilled a 26 diameter hole to 120. Express installed 115 of 20 OD casing. During drilling, water was encountered at a coal seam at an approximate depth of 47 to 49. Unconsolidated rocks were encountered just below the coal seam at an approximate depth of 49 to 52. Allied Oil & Gas Field Services was the cementing contractor. The cement was mudscaled at 15.4 pounds per gallon (ppg). Allied adjusted the density of the cement and pumped 63.5 bbls of cement slurry at 15.6 ppg. The cement was displaced with 39 bbls of water. Cement returns did not reach the surface. The water level, and subsequently the cement level, never came above the coal seam and unconsolidated rocks. Approximately 18 cubic yards of redi-mix with #8 limestone was used to cement the conductor casing to surface, and to complete the floor of the cellar.
0925256486 2013-01-17 Drill / Deepen / Reopen No Violations I went to the site to witness the cementing of the surface casing. The drilling contractor, Nomac 39, spudded the well on 01/16/2013. The 17.50 diameter surface hole was drilled to 658. There were no issues while drilling the surface hole. Circulation was maintained throughout the drilling. The 13.375 OD surface casing was set at 658 KB. A total of seven centralizers were installed on the surface casing string. Halliburton was the cementing contractor. There was good fluid circulation throughout the cementing operation. The cement was mudscaled at 16.1 lbs per gallon (ppg), then 15.6 ppg, then 15.5 ppg. Halliburton adjusted the cement density, and pumped 138 bbls of cement slurry at 15.6 ppg. Cement returns to surface were observed. An estimated 33 bbls of cement returned to surface. The plug was displaced with 93.5 bbls of water. The plug bumped the float collar at 5:14pm. Halliburton pressured up on the casing, but cement head plug holder seal leaked and pressure could not be maintained. The float did hold. Approximately 0.5 bbls of water bled back. The pressure test on the casing and shoe will be conducted before initiating drilling of the intermediate hole.
1358527853 2013-01-17 Drill / Deepen / Reopen No Violations I was onsite with Rick Anderson to witness the beginning of the surface cement job for Nomac 39. Around 30 barrels of cement were circulated to surface. During the pressure test of the plug a seal broke and pressure had to be bleed off. The floats did indeed hold and the cement and shoe will be pressure tested when it sets.
1359471319 2012-09-24 Surface Facility Construction No Violations I went to the site to review the construction process of the drilling pad. The entry was started with rock, and the access road was cut, but not rocked. The drilling pad was cut. A construction foreman indicated that stabilization of the pad would begin the next day (09/25/2012).
1360272503 2013-02-04 Drill / Deepen / Reopen No Violations

-based mud used to drill the hole and condition the hole, respectively, a lead cement followed by a tail cement were pumped. The lead cement was mudscaled, and the density adjusted. Halliburton pumped 172.7 bbs of lead cement at 14.2 ppg. The tail cement was mudscaled, and the density adjusted. Halliburton pumped 256 bbls of tail cement at 15.6 ppg. The plug was dropped, and displaced with 299.6 bbls of water. The plug bumped the float collar at 9:28am on 02/04/2013. Pressure held. The float held when the pressure was released. 4.0 bbls of water bled back.

-based mud (9.4 pounds per gallon) for approximately 18 hours to kill the gas before cementing the intermediate casing. The gas flow was stopped prior to cementing the intermediate casing.

1668147339 2017-08-21 Production Wells No Violations The purpose of my inspection was to conduct a status check on well#2-1754. The lease road and well pad remained in good repair. A locked chain link fence was constructed around the facility. No signs of vandalism were present. A stable earthen berm was present outside of the fenced perimeter. Correct identification was present. The well cellar contained fresh water near free board. No leaks were detected. The 9.625 annular pressure gauge indicated 0 psi. The 5.5 annular pressure gauge indicated 200 psi. The well was flowing during the inspection. The 5.5 production casing was equipped with a plunger lift assembly. Production equipment on the pad consisted of (1) sand knockout and GPU specific to 2-1754. Other equipment shared with well #2-1753 consisted of one (1) hydrate inhibitor tank, one (1) meter run, one (1) thermal generator and two (2) 400 bbl produced water tanks inside steel containment. The containment was lined with an impervious liner and was holding fresh water. No violations were observed during this inspection.
