0164601527 2013-01-12 Drill / Deepen / Reopen No Violations I went to the site to witness the cementing of the production casing. The drilling contractor, Nomac 39, drilled a 8.75 diameter hole from below the base of the intermediate casing at 1808 to 7237, then a 8.5 diameter hole to 8,080, then a 7.875 diameter hole to the total measured depth of 13,723. Total depth was reached on 01/11/2013 at 6:15am. There were no issues while drilling. 5.50 OD casing was set at 13,723 KB. A total of 76 centralizers were installed on the production casing. Halliburton was the cementing contractor. There was good fluid circulation throughout the cementing operation. The cement job was done in stages. Following two spacers designed to recover the oil-based mud used to drill the hole and condition the hole, respectively, a lead cement followed by a tail cement were pumped. The lead cement was mudscaled at 14.5 and 14.3 lbs per gallon (ppg). Halliburton adjusted the density of the cement and pumped 164 bbs of lead cement at 14.2 ppg. The tail cement was mudscaled at 15.6 ppg. Halliburton pumped 253 bbls of tail cement at 15.6 ppg. The plug was dropped, and displaced with 300 bbls of water. The plug bumped the float collar at 8:37am on 01/13/2013. Pressure held. The float held when the pressure was released. 4.5 bbls of water bled back.
0594869792 2012-10-25 Drill / Deepen / Reopen No Violations I witnessed the cementing of the conductor casing. Express Energy Services drilled a 26 diameter hole to 120. Express installed 117 of 20 OD casing. During drilling, water was encountered at a coal seam at an approximate depth of 47 to 49. Unconsolidated rocks were encountered just below the coal seam at an approximate depth of 49 to 52. Allied Oil & Gas Field Services was the cementing contractor. The cement was mudscaled at 15.5 pounds per gallon (ppg). Allied adjusted the density of the cement and pumped 63.5 bbls of cement slurry at 15.6 ppg. The cement was displaced with 40.8 bbls of water. Cement returns did not reach the surface. The water level, and subsequently the cement level, never came above the coal seam and unconsolidated rocks. Approximately 18 cubic yards of redi-mix with #8 limestone was used to cement the conductor casing to surface, and to complete the floor of the cellar.
1071737761 2012-12-27 Drill / Deepen / Reopen No Violations I went to the site to witness the cementing of the intermediate casing. The drilling contractor, Nomac 39, drilled a 12.25 diameter hole to 1818. There were no issues while drilling the intermediate hole. 9.625 OD casing was set at 1808 KB. A total of 15 centralizers were installed on the intermediate casing string. Halliburton was the cementing contractor. There as good fluid circulation throughout the cementing operation. The cement was mudscaled at 15.8 lbs per gallon (ppg). Halliburton adjusted the cement density, and pumped 141 bbls of cement slurry at 15.6 ppg. Cement returns to the surface were observed. An estimated 21 bbls of cement returned to the surface. The plug was displaced with 131 bbls of water. The plug bumped the float collar at 2:14am on 12/28/2012. Pressure held. The float held when the pressure was released. 1.0 bbls of water bled back.
1150456142 2013-01-07 Drill / Deepen / Reopen No Violations I stopped at the site to check on the progress of the drilling. The rig was tripping the drill pipe in with their lateral assembly. Current total measured depth was at 8080.
1249776329 2017-01-24 Production Wells No Violations The purpose for my inspection was to conduct a status check on well #2-1753-00-00. RBDMS indicates the operator is current with reporting production. The lease entrance is marked with a sign board containing signs reporting required well identification and emergency response information. The lease road is also utilized by the surface owner and was stable and drivable. No signs of erosion were observed adjacent to the road traveling to the pad. An earthen dike perimeter is erected around the well pad. A chain link fence with barbed wire top is constructed around the pad to prevent unauthorized entry. An Emergency Shut Down (ESD) button was adjacent to the main entrance gate. The main entry gate and escape doors positioned around the perimeter were found to be closed and locked. The pad surface was composed of crushed gravel and remained in good repair. I observed water ponding along the outer edge of the pad adjacent to the berm. The soil stockpile was stabilized with well-
1359120530 2012-09-24 Surface Facility Construction No Violations I went to the site to review the construction process of the drilling pad. The entry was started with rock, and the access road was cut, but not rocked. The drilling pad was cut. A construction foreman indicated that stabilizing at the pad would begin the next day (09/25/2012).
1359122683 2012-10-24 Drill / Deepen / Reopen No Violations I was at the site checking the progress of the drilling of the hole for the conductor casing. Express Energy Services was on site drilling the conductor hole. Express was also drilling the conductor for the Milltree 8H. Express expected to finish the holes today, and install and cement the conductor casing the next day (10/25/2012).

-lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. No site drains were observed on the pad. Outside the fence, there is an earthen berm, with well-established ground cover. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. No Violations observed.

1503320818 2017-08-21 Production Wells No Violations The purpose of my inspection was to conduct a status check on well#2-1753. The lease road and well pad remained in good repair. A locked chain link fence was constructed around the facility. No signs of vandalism were present. A stable earthen berm was present outside of the fenced perimeter. Correct identification was present. The well cellar contained fresh water near free board. No leaks were detected. The 9.625 annular pressure gauge indicated 0 psi. The 5.5 annular pressure gauge indicated 480 psi. Stainless steel tubing was connected from the valve to the downstream flow side of the well head to release pressure into the production unit. The well was flowing during the inspection. The 5.5 production casing was equipped with a plunger lift assembly. Production equipment on the pad consisted of (1) sand knockout and GPU specific to 2-1753. Other equipment shared with well #2-1754 consisted of one (1) hydrate inhibitor tank, one (1) meter run, one (1) thermal generator and two (2) 400 bbl produced water tanks inside steel containment. The containment was lined with an impervious liner and was holding fresh water. No violations were observed during this inspection.

Outside the fence, there is an earthen berm with fairly good ground cover. Some areas of the berm are shaley with sparse growth. The soil stockpile is well stabilized with ground cover. The area of disturbance surrounding the pad has good ground cover. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete.
