I emailed Steve Ault of Chesapeake on 4/24/2015 about the identification signs. He replied on 4/27/2015 that they had located the wellhead identification signs and they would be installed within the week. I recommend temporary inactive status be granted.

0814327826 2016-06-21 Production Wells No Violations My purpose for inspection was to conduct a second year temporary inactive well status inspection.

There is one well on the pad. The sign board posted at the lease entrance displayed signs with the well name, address, permit number and emergency contact information. The lease road was graveled and in good repair with a locked gate at Low Road. Erosion and sedimentation controls were in place and ground cover was well established along the road. I walked the entire location and found the well pad constructed with an earthen berm perimeter in good repair, being stabilized with well-established vegetation. Erosion and sedimentation controls are in place and functioning. The well pad was found to be enclosed by a chain link fence with barbed wire at the top. The primary entrance and escape doors were found locked. The surface of the well pad consists of gravel and is in good repair. A perimeter drain was installed and and the stand pipe was found open. Soil stockpile is stabilized with well-established vegetation. The Chamberlain 5H was constructed with 140 of 20 conductor, 590 of 13 3/8 surface casing, 1499 of 9 5/8 intermediate casing and 13,981 of 5 1/2 production casing. Driling was completed 10/25/2012 and stimulated on 11/7/2012. The well head was constructed with a gate valve assembly and found to be protected by a perimeter of steel cattle gates further surrounded by concrete Jersey barriers. Proper well identification signage was displayed. A cellar grate was in place to prevent falls. The cellar contained water with approximately 10 inches of freeboard. The production casing pressure gauge indicated 2900 psi. No other pressures could be determined due to the water level in the cellar and the gauge faces being covered with algae. No sheen or leaking gas was observed on the water in the cellar. The mousehole was cemented. Gate valves are chained and locked. No other production equipment is on the pad. 1256709192 2016-12-06 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1751. The well is currently in temporary inactive status. Correct well identification remains present at the entrance and well head. Lease access and the well pad facilities remain in good repair. No other equipment is on the pad. The Owner raised and identified the 9.625 intermediate annular casing pressure gauge and the 5.5 production annular casing pressure gauge. The intermediate annular casing gauge indicated 0 psi. The production casing annular gauge indicated 0 psi. Maximum allowable annular surface pressure (MAASP) at the surface per ORC 1501:9-1-08 (N) (2) is 454 psi. The production casing valve was closed. I will continue to monitor casing pressures on a regular basis to ensure compliance.

1444585578 2013-10-01 Preliminary Restoration No Violations I reviewed the site to assess the completion of preliminary restoration. There is a sign board at the access road entry featuring the well name, permit number and emergency contact information. The access road has a padlocked gate a short distance from Low Road. The access road is in good repair. Drainage and erosion controls were in place and appear to be functioning. Ground cover was well established along the access road. I found the drilling/production pad to be enclosed entirely with a chain link fence with barbed wire at the top, and with a padlocked gate. The pad has a crushed limestone surface, and is in good repair. There is no pad drainage system installed. There is some standing water in the northeast corner, and along the north, northwest and west perimeter, and the south perimeter inside the pad. I walked the exterior perimeter and noted that the berm is in good repair and ground cover is fairly well established. There are some areas that cover is not well established, but there is no significant erosion taking place. The area of disturbance around the pad has decent ground cover. Again, some areas are thinner, but there is no significant erosion taking place. The soil stockpile is well stabilized with vegetation. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The well has been hydraulically fractured, and has a partial Christmas tree valve assembly. There are bull gates and concrete barriers surrounding the wellhead and cellar to prevent vehicle impacts. There are grates over the cellar to prevent falls. There is no ID tag on the well, but it is the only one on the site. The cellar is full of water to the top. There is no production equipment at the site. The mouse hole has not been plugged, but has a steel plate welded on top. Preliminary restoration approval will not be granted pending installation of production equipment and re-inspection.

FTS International was on site for the stimulation, and completion consultant David Smith was supervising. I witnessed the stage, and the job went as planned. There was no unexpected annular pressure on the surface and intermediate casings.

The access road, pad and erosion and sedimentation controls were in good repair. The security fence was locked. No signs of vandalism were observed. Correct identification was present. The well head was protected by concrete jersey barriers. The cellar was open and contained water near free board. Annular pressure gauges were plumbed above grade and identified. The 9.625 and 5.5 valves were open. The 9.625 and 5.5 annular gauges both indicated 0 psi. The well head was equipped with a Christmas tree valve assembly. Valves were closed, chained and locked. No leaks were detected. The well remained in temporary inactive status. No violations were observed during this inspection.

There is no tanks or production unit at this time. The well is equipped with a Christmas tree. Values are locked out. I walked the entire site, and found the location to be in good condition. All erosion and sedimentation controls, and drainage controls appear to be functioning. Ground cover is well established along the access road and around the drilling pad. No areas of erosion are present. Preliminary restoration approval will not be finalized at this time.

The road and well pad remained in good repair. The perimeter security fence remained in good repair and locked. The perimeter slopes remained stable with well-established vegetation. The French drain stand pipes remained open. Correct identification was present at the well head. Jersey barriers were present to prevent a collision. Grates remained over the cellar to prevent falls. The cellar contained freshwater. No leaks were observed. Gauges to measure annular casing pressure were present and labeled. The 9.625 annulus indicated 0 psi. The 5.5 annulus indicated 0 psi. The 5.5 production casing wing valve gauge indicated 0 psi. The well head was equipped with a night cap and observation port. All valves were closed, chained and locked. No production equipment was on location. No violations were observed during this inspection.
