0313990817 2016-06-21 Production Wells No Violations My purpose for inspection was to conduct a second year temporary inactive well status inspection.

There is one well on the pad. The sign board posted at the lease entrance displayed signs with the well name, address, permit number and emergency contact information. The lease road was graveled and in good repair with a locked gate at State Route 7. Erosion and sedimentation controls were in place and ground cover was well established along the road. I walked the entire location and found the well pad constructed with an earthen berm perimeter in good repair, being stabilized with well-established vegetation. Erosion and sedimentation controls are in place and functioning. The well pad was found to be enclosed by a chain link fence with barbed wire at the top. The primary entrance and escape doors were found locked. The surface of the well pad consists of gravel and is in good repair. Soil stockpile is stabilized with well-established vegetation. The Lehman 8H was constructed with 120 of 20 conductor, 758 of 13 3/8 surface casing, 1527 of 9 5/8 intermediate casing and 14196 of 5 1/2 production casing. Drilling was completed 11/20/2012. The well was stimulated 3/5/2013 and completed with 7872 of 2 7/8 tubing. The well head was constructed with a Christmas tree valve assembly and found to be protected by a perimeter of steel cattle gates further protected by concrete Jersey barriers. Proper well identification signage was displayed. A cellar grate was in place to prevent falls. The cellar contained water with approximately 4 feet of freeboard. The production tubing and casing pressure gauges indicated 0 psi. Due to the position of the intermediate casing gauge, I was unable to determine what pressure the gauge was indicating. Also, the surface casing gauge was covered with debris preventing an accurate indication of pressure on the gauge. No sheen or leaking gas was observed on the water in the cellar. 0562527426 2014-06-17 Preliminary Restoration No Violations I stopped by the site to check on the status of well completion and restoration. The well has been hydraulically fractured. There is no tanks or production unit at this time. The well is equipped with a Christmas tree. Values are locked out. I walked the entire site, and found the location to be in good condition. All erosion and sedimentation controls, and drainage controls appear to be functioning. Ground cover is well established along the access road and around the drilling pad. No areas of erosion are present. Preliminary restoration approval will not be finalized at this time.
0757675359 2013-08-20 Preliminary Restoration No Violations I reviewed the site to assess the completion of preliminary restoration. There is a sign board at the access road entry featuring the well name, permit number and emergency contact information. The access road has a padlocked gate near State Route 7. The access road is in good repair. Drainage and erosion controls were in place and appear to be functioning. Ground cover was well established along the access road. I found the drilling/production pad to be enclosed entirely with a chain link fence with barbed wire at the top, and with a padlocked gate. The pad has a crushed limestone surface, and is in good repair. The pad does have some ponded water along the perimeter. The fence is in good repair. Outside the fence, there is an earthen berm. I walked the exterior perimeter and noted that the berm is generally in good repair and ground cover is generally well established. There are some areas of the berm that do not have much ground cover due to being shaley with little soil. Most of the area of disturbance has good ground cover. The soil stockpile is well stabilized with vegetation. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The well has been fracked, and has a Christmas tree valve assembly. There are bull gates and concrete barriers surrounding the wellhead and cellar to prevent vehicle impacts. There are grates over the cellar to prevent falls. The mouse hole from drilling is still present, and has not been plugged. There is no production equipment at the site. The cellar is full of water to the top. Preliminary restoration approval will not be finalized pending installation of production equipment and re-inspection.
1036433271 2016-02-26 Production Wells No Violations I was on location for a status check. My inspection found the access road gate locked and the

access road in good repair. The pad is fenced and all man doors were locked. An identification sign is posted at the access road entrance and well. The #8H is not in production. The well is equipped with a christmas tree valve assembly. The equipment on the pad consists of the following:

The well head was protected by concrete jersey barriers. The cellar was open and contained water near free board. Annular pressure gauges were plumbed above grade and identified. The 9.625 and 5.5 valves were open. The 9.625 annular gauge indicated 0 psi. The 5.5 annular gauge indicated 260 psi. The well head was equipped with a Christmas tree valve assembly. Valves were closed, chained and locked. No leaks were detected. The well remained in temporary inactive status. No violations were observed during this inspection.
1704832102 2018-11-20 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1749. Correct identification was present at the entrance. The road and well pad remained in good repair. The perimeter security fence remained in good repair and locked. The perimeter berm and slopes remained stable with well-established vegetation. The French drain stand pipes remained open. Correct identification was present at the well head. Jersey barriers were present to prevent a collision. Grates remained over the cellar to prevent falls. The cellar contained freshwater. No leaks were observed. Gauges to measure annular casing pressure were present and labeled. The 9.625 annulus indicated 0 psi. The 5.5 annulus indicated 240 psi. The 5.5 production casing wing valve gauge indicated 0 psi. The production tree cap gauge indicated 0 psi. All valves were chained, closed and locked. No production equipment was on location. No violations were observed during this inspection.

I emailed Steve Ault of Chesapeake on 4/24/2015 about the identification signs. Correct well identification remains present at the entrance and well head. Lease access and the well pad facilities remain in good repair. No other equipment is on the pad. The Owner raised and identified the 9.625 intermediate annular casing pressure gauge and the 5.5 production annular casing pressure gauge. The intermediate annular casing gauge indicated 0 psi. The production casing annular gauge indicated 260 psi. Maximum allowable annular surface pressure (MAASP) at the surface per ORC 1501:9-1-08 (N) (2) is 462 psi. The production casing valve was closed. I will continue to monitor casing pressures on a regular basis to ensure compliance.
