0205284574 2018-03-08 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1730. ODNR-DOGRM Northeast District Supervisor Stephen Ochs was present during the inspection. The well had a valid permit for temporary inactive status. Correct identification was present at the entrance. The road and well pad remained in good repair. The perimeter security fence remained in good repair and locked. The topsoil stock pile remained stable. The cut and fill slopes remained stable with no evidence of mass wasting. The earthen berm perimeter remained stable. The standpipe drains on the North and South side were shut. Fresh water was ponding on the pad. Correct identification was present at the well head. Jersey barriers were present to prevent a collision. Grates remained over the cellar to prevent falls. The cellar contained freshwater. No leaks were observed. Gauges to measure annular casing pressure were in the open position and labeled. The 9.625 annulus indicated 0 psi. The 5.5 annulus indicated 500 psi. The 5.5 production casing wing valve gauge indicated 600 psi. The production tree cap gauge indicated 0 psi. No production equipment was on location. No violations were observed during this inspection.

-established ground cover. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. I emailed Steve Ault of Chesapeake on 4/24/2015 about the identification signs. He replied on 4/27/2015 that they had located the wellhead identification signs and they would be installed within the week. I recommend temporary inactive status be granted.

0343332951 2016-04-04 Production Wells No Violations I was on location for a status check. My inspection found the access road gate locked and the

access road in good repair. The pad is fenced and all man doors were locked. An identification sign is posted at the access road entrance and well. The #6H is not in production. The well is equipped with a christmas tree valve assembly. The equipment on the pad consists of the following:

0349105388 2016-11-29 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1730-01. The well is currently in temporary inactive status. Correct well identification remains present at the entrance and well head. Lease access and the well pad facilities remain in good repair. No other equipment is on the pad. The Owner raised and identified the 9.625 intermediate annular casing pressure gauge and the 5.5 production annular casing pressure gauge. The intermediate annular casing gauge indicated 0 psi. The production casing annular gauge indicated 640 psi. Maximum allowable annular surface pressure (MAASP) at the surface per ORC 1501:9-1-08 (N) (2) is 390 psi. The production casing pressure gauge indicated 400 psi. I will be contacting Chesapeake Regulatory Affairs Advisor Jody Jones concerning the observed pressure on the production casing annulus in order to facilitate further investigation.
0458046771 2017-08-03 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1730-01. The lease road and pad remained in good repair. A locked chain link perimeter fence was present around the pad. Correct identification was present at the well head. The well cellar contained fresh water at free board and was covered by a steel grate. No leaks were detected. The annular pressure gauges were identified. The 9.625 pressure gauge indicated 0 psi. The 5.5 annular pressure gauge indicated 710 psi. No pressure relief device was present on the 9.625 casing. The 5.5 production casing was equipped with a Christmas tree valve assembly. Valves were closed. The 5.5 production casing gauge indicated 400 psi. The tubing pressure gauge indicated 0 psi. No other equipment was present on the pad. I will contact Chesapeake regulatory manager Jody Jones concerning the excessive annular pressure.
0988798440 2016-06-16 Production Wells No Violations My purpose for inspection was to conduct a second year temporary inactive well status inspection. The inspection was conducted with ODNR DOGRM Northeast Regional Supervisor Thomas Hill and Northeast District Supervisor Richard Anderson. There is one well on the pad.

The sign board posted at the lease entrance displayed signs with the well name, address, permit number and emergency contact information. The lease road was graveled and in good repair with a locked gate off of Miller Road. Erosion and sedimentation controls were in place and ground cover was well established along the road. I walked the entire location and found the well pad constructed with an earthen berm perimeter in good repair, being stabilized with well-established vegetation. Erosion and sedimentation controls are in place and functioning. The well pad was found to be enclosed by a chain link fence with barbed wire at the top. The primary entrance and escape doors were all found to be locked. The surface of the well pad consists of gravel and is in good repair. The well pad was constructed with a site drain and the standpipe was shut. Soil stockpile is stabilized with well-established vegetation. The Grubbs 6H was constructed with 90 of 20 conductor, 615 of 13 3/8 surface casing, 1289 of 9 5/8 intermediate casing and 11000 of 5 1/2 production casing. Drilling was completed 7/2/2012. The well was stimulated on 7/19/2012 and completed with 7796 of 2 7/8 tubing. The well head was constructed with a Christmas tree valve assembly and found to be protected by a perimeter of steel cattle gates further protected by concrete Jersey barriers. Proper signage was displayed. A cellar grate was in place to prevent falls. The cellar contained water that was near freeboard. No leaking gas was observed in the cellar. The mousehole was cemented. Gate valves are chained and locked. No other production equipment is on the pad. I recommend that the pressures associated with the respective

1344444219 2012-07-30 Drill / Deepen / Reopen Violations Noted On 7/30/2012, I was contacted by the ODNR Uniontown Office that Ray Daniels was driving south on State Route 11 when he saw a flame coming from what he thought was a drilling rig. When I arrived in the area of Route 517 and Davnor Road, I saw a flame coming from a Chesapeake Location ( Grubb 6-11-2 6 ). I spoke to a person working for Chesapeake he said that they were doing a flow test and that they would be flaring the gas until 8/8/2012. The ODNR Uniontown Office was not notified.
