0921434621 2016-03-29 Production Wells No Violations I was on location for a status check. My inspection found the access road gate locked and the

access road is in good repair. An identification sign at the access road entrance, well, and tank battery. The pad is fenced and all man doors are locked. The #6H well is in production. The well is equipped with a christmas tree valve assembly. There is a steel dike around the tank battery. The equipment on the pad consists of the following: 6 - Storage tanks 400 bbl 1 - Vapor recovery unit (VRU) 2 - Gas processing units (GPU) 1 - Vertical separator 2 - Sand separators 1 - Thermal electric generator 2 - Producing wells Site fenced and man doors locked

-lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Along the interior of the chain link fence there is a site drain installed. Outlets are located at the south and west ends of the pad. Inside the fence, there is an earthen berm, with well-established ground cover. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete and approved.

2146846401 2017-08-30 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1720. The lease road and pad surface was composed of crushed gravel and remained in good repair. The topsoil stockpile remained stable. Correct identification was posted at the entrance and well head. A locked chain link fence remained around the pad. The well head was protected by jersey barriers. The cellar was empty and contained gravel. Steel grating covered the cellar to prevent falls. Annular gauges were open and plumbed above grade and identified. The 9.625 gauge was digital and not indicating a pressure. The 5.5 annulus gauge identified 0 psi. The 5.5 production casing was equipped with a plunger lift assembly and was in production. No leaks were observed. Production equipment on the pad consisted of one (1) sand knock out and GPU, one (1) hydrate inhibitor tank, one (1) thermal generator, one (1) meter run and one (1) VRU. The tank battery consisted of three (3) 400 bbl produced water tanks and three (3) condensate tanks inside containment. The containment was lined with impervious material and was holding water with no sheen. Valves were closed and plugged. No violations were observed during this inspection.
