I emailed Steve Ault of Chesapeake on 4/24/2015 about the identification signs. He replied on 4/27/2015 that they had located the wellhead identification signs and they would be installed within the week. I recommend temporary inactive status be granted.

0110020991 2016-03-03 Production Wells No Violations I was on location for a status check. My inspection found the access road gate locked and the access road in good repair. The pad is fenced and all man doors were locked. An identification sign is posted at the access road entrance and well. The #1H is not in production. The well is equipped with a christmas tree valve assembly.

0318048334 2016-06-21 Production Wells No Violations My purpose for inspection was to conduct a second year temporary inactive well status inspection.

There is one well on the pad. Drilling was completed 8/30/2012. The well was stimulated 1/31/2013. The sign board posted at the lease entrance displayed signs with the well name, address, permit number and emergency contact information. The lease road was graveled and in good repair with a locked gate at Peace Valley Road. Erosion and sedimentation controls were in place and ground cover was well established along the road. I walked the entire location and found the well pad constructed with an earthen berm perimeter in good repair, being stabilized with well-established vegetation. Erosion and sedimentation controls are in place and functioning. The well pad was found to be enclosed by a chain link fence with barbed wire at the top. The primary entrance and escape doors were found locked. The surface of the well pad consists of gravel and is in good repair. The pad is constructed with a perimeter drain and both standpipes were found to be shut. Soil stockpile is stabilized with well-established vegetation. The well head was constructed with a Christmas tree valve assembly and found to be protected by a perimeter of concrete Jersey barriers enclosing steel cattle gates. Well identification signage was displayed. A cellar grate was in place to prevent falls. The cellar contained water with approximately 1 foot of freeboard. The tubing pressure gauge indicated 0 psi. Due to the water level in the cellar, I was unable to see any other gauges. No sheen or leaking gas was observed on the water in the cellar. The mousehole was cemented. Gate valves are chained and locked. No other production equipment is on the pad. I request that the water level in the cellar be drawn down to a level where annular pressures can be determined and monitored while the well is in inactive status.

0489493311 2012-11-06 Preliminary Restoration No Violations I stopped at the site for a status check. There was no activity at site, and the gate and fence entry were locked.
0555413997 2018-11-20 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1718-01. Correct identification was present at the entrance. The road and well pad remained in good repair. The perimeter security fence remained in good repair and locked. The topsoil stockpile was opened up and material was borrowed. The perimeter slopes remained stable with well-established vegetation. The French drain stand pipes remained closed. Correct identification was present at the well head. Jersey barriers were present to prevent a collision. Grates remained over the cellar to prevent falls. The cellar contained freshwater. No leaks were observed. A tree continued to grow adjacent to the cellar. Gauges to measure annular casing pressure were present and labeled. The 9.625 annulus indicated 35 psi. The 5.5 annulus indicated 350 psi. The 5.5 production casing wing valve gauge indicated 0 psi. The production tree cap gauge indicated 0 psi. All valves were closed, chained and locked. No production equipment was on location. No violations were observed during this inspection.

0859744320 2012-09-01 Casing String Witnessed No Violations I went to the site to witness the cementing of the production casing. The drilling contractor, Patterson 154, drilled an 8.50 diameter hole from the kick--based mud used to drill and condition the hole both a lead cement followed by a tail cement were pumped. The cement was mudscaled and Halliburton pumped 331 bbls of lead slurry at 14.4 pounds per gallon (ppg), followed by 370 bbls of tail cement at 15.6 ppg. The plug was dropped and displaced with 337 bbls of water. The plug bumped the float collar at 5:02am on 09/02/2012. Pump pressure at plug bump was 2950 psi, and it was increased to 3400 psi and held for five minutes. Pressure held. The float held when the pressure was released and 3.0 bbls of water bled back.

Following completion of the pilot hole and subsequent testing, Halliburton was on site to plug back the well, and set a kick off plug. Three plugs were set. 1296404646 2016-12-06 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1718. The well is currently in temporary inactive status. Correct well identification remains present at the entrance and well head. Lease access and the well pad facilities remain in good repair. No other equipment is on the pad. The Owner raised and identified the 9.625 intermediate annular casing pressure gauge and the 5.5 production annular casing pressure gauge. The intermediate annular casing gauge indicated 0 psi. The production casing annular gauge indicated 260 psi. Maximum allowable annular surface pressure (MAASP) at the surface per ORC 1501:9-1-08 (N) (2) is 477 psi. The production casing valve was closed. I will continue to monitor casing pressures on a regular basis to ensure compliance.
1346761656 2012-08-13 Drill / Deepen / Reopen No Violations ON 8/13/20912, I was on location to meet Tim Allen Drilling Foreman on Patterson Rig 154.They were preparing to log the well bore before running there 9.625 intermediate casing. I arrived on location at 7:00 pm they ran 1577 40 # 9.625 intermediate casing. Halliburton on location to cement the casing good circulation and good cement returns.

The well was stimulated 1/31/2013 and completed with 7196 of 4.5 tubing. The well head was constructed with a Christmas tree valve assembly and found to be protected by a perimeter of steel cattle gates further protected by concrete Jersey barriers. Proper well identification signage was displayed. A cellar grate was in place to prevent falls. The cellar contained water with approximately 1 foot of freeboard. The tubing pressure gauge indicated 0 psi. Due to the water level in the cellar, I was unable to see any other gauges. No sheen or leaking gas was observed on the water in the cellar. The mousehole was cemented. Gate valves are chained and locked. No other production equipment is on the pad. I recommend

No leaks were detected. The well remained in temporary inactive status. No violations were observed during this inspection.
