0792970721 2017-01-24 Production Wells No Violations The purpose for my inspection was to conduct a status check well #2-1705-01-00). RBDMS indicates the operator is current with reporting production. The lease entrance is marked with a sign board containing signs reporting required well identification and emergency response information. At the time of my inspection, the local area had received a coating of snow that covered the location. The lease road is drivable. No signs of erosion were observed adjacent to the road traveling back to the pad. The soil stockpile remains stable with well-

1418410107 2014-12-10 Preliminary Restoration No Violations

-lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. There is an earthen berm, with well-established ground cover. The soil stockpile is well stabilized with vegetation. Crops, woodlands and/or underbrush are growing normally outside the area of disturbance. Drainage and erosion controls outside the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete and has passed inspection.

1919512571 2017-08-29 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1705. The lease road and pad surface was composed of crushed gravel and remained in good repair. Correct identification was posted at the entrance and well head. A locked chain link fence remained around the pad. Ponding was present in the Southeast corner of the pad. The well head was protected by jersey barriers. The cellar contained freshwater with no sheen. Steel grate covered the cellar to prevent falls. The annular gauges were plumbed at grade and identified. Water in the cellar prevented determining if the valves were open. The 9.625 gauge indicated 0psi. The 5.5 annulus gauge identified 40 psi. The 5.5 production casing was equipped with a plunger lift assembly and was in production. The production casing gauge indicated 595 psi. The tubing pressure gauge indicated 130 psi. No leaks were observed. Production equipment on the pad consisted of one (1) sand knock out and GPU, one (1) hydrate inhibitor tank, one (1) thermal generator, one (1) meter run and two (2) 400 bbl production fluid tanks in containment. The containment was lined with impervious material and was holding water with no sheen. Valves were closed and plugged. No violations were observed during this inspection.
