0855313955 2014-10-22 Preliminary Restoration No Violations I was on site to assess the completion of preliminary restoration. All drilling equipment has been removed and production equipment is in place. Identification was posted on the tank battery. The tank battery consists of one 100 bbl production tank, no brine tank, and one horizontal separator. The tank valves are plugged. There is a meter box with a current chart showing production. A dike is present and in good repair. The wellhead is equipped with a plunger lift and a domestic supply line. Vegetation is growing on all disturbed surfaces. The access road and site are stable. Preliminary restoration is complete and approved.
1265227888 2010-02-03 Drill / Deepen / Reopen No Violations I witnessed the cementing of the 8 5/8casing. There were no violations at this time.
1265393061 2010-02-03 Drill / Deepen / Reopen No Violations I performed a rig inspection on G&H drilling rig #91. The drilling pits and pit liners were in good condition. The Permit was posted. There were no violations at this time.
1449868068 2017-12-11 Production Wells No Violations The purpose of my inspection was conduct a status check on well #2-1689. A review of RBDMS indicated production was current. Lease access and the locations were stable. The 8.625 well head was plumbed above grade and open to atmosphere. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. Gas was observed leaking from the tubing head assembly. A domestic supply connection was present. A stable earthen dike that was free of fluid was present around the production equipment. Equipment within the dike one (1) horizontal separator and one (1) 100 bbl production tank. The separator remained in good repair and contained a current 7 day production chart. The production tank remained in good repair and contained correct identification. Valves were closed and plugged. I called and spoke with lease operator Russell Miller Jr. concerning the well head leak. Mr. Miller reported he would get the well produced down to make repairs. I will schedule a follow-up inspection to document the status of repair.
