0125775983 2012-01-30 Preliminary Restoration No Violations On 1/30/2012, I performed a status check on the above well site. This well is in production, the site is reclaimed and idenfication signs are posted. The Preliminary Restoration is PASSED.
0578075314 2016-10-24 Production Wells No Violations The purpose of my inspection was to conduct a status check of well #2-1642. A review of RBDMS indicates the last reported production was in 2013. The lease road to access the well is utilized for agriculture and remains stable and drivable. The well location is within an agriculture field that and remains stable. Correct and legible identification is posted at the well head. The 8.625 surface casing is below grade. The 4.5 production casing is equipped with a plunger lift assembly. Production valves are in the closed position. No audible or visual leaks were detected. No domestic supply connection is present. The tank battery has been removed. Following the inspection, I met lease operator Steve Pegg while inspecting offsetting well # 2-1643. Mr. Pegg indicated that well #2-1642 had not been operated for some time and that the tank battery had been removed at the request of the electric utility due to its position under the high-tension wires. I contacted Enervest Superintendent Tim McConaha to inquire about the Operators plans for the well. Mr. McConaha was unsure of what Enervest had planned for the well and stated he would investigate further and communicate back with his findings.

The lease access road is drivable and in good repair. The well location is within a field and is stable. Proper identification is present at the well head. The 8.625 casing head is below grade. The 4.5 production casing is equipped with a plunger lift assembly. Production valves are in the closed position. No audible or visual leaks were detected. No domestic supply connection is present. Per a field conversation with lease operator Steve Pegg on 10/24/2016 the production equipment has been removed. I spoke with Enervest Field Superintendent Tim McConaha concerning the well on 10/26/2016. Tim is unsure of the status of the well and communicated he would investigate the situation. He and I agreed to speak during the week beginning 10/31/2016 to discuss Enervests plans for the well.
0623646976 2017-10-19 Plug / Plug Back No Violations The purpose of my inspection was to follow-up on compliance notice # 1597981365. The remainder of plugging activates occurred on 10/18/2017. The lease access remained stable. The former location had been graded to local contour. The earthen pit was backfilled. I met Enervest Northern Ohio Superintendent Doug Beighley on location. I received the cement tickets from Chipco cement truck operator Skip Baker as Chipco was removing equipment from location. Mr. Beighley communicated to me that he or Tim McConaha would be submitting a Form 5 in conjunction with restoration activities. I will schedule to re-inspect well #2-1642 once the Form 5 is received. No further action is necessary.

1344693247 2017-11-22 Final Restoration No Violations The purpose of my inspection was to document the status of final restoration on well #2-1642. The well owner obtained a Form 5 landowner waiver allowing the flow line and sales line in place. Access to the former well location was to remain in place to facilitate access to well #2-1643. The earthen pit excavated was backfilled. The location was within a field utilized for agricultural purposes and was graded back to local contour in preparation for future cultivation. No erosion or sedimentation controls were necessary. Production equipment was shared with #2-1643 and remained in place. Any debris was removed. The location meets the requirements of final restoration.
1508168013 2017-10-16 Plug / Plug Back No Violations The purpose of my inspection was to witness plug and abandonment activities on well #2-1642. Chipco was the plugging contractor. Chipco was on location rigging up Rig #5. The crew proceeded to blow down the well and proceeded to disassemble the well in preparation to remove the tubing from the well. Access to the well remained stable. No earthen pit was dug yet. I will schedule to be witness the setting of plugs on 10/17/2017.
1597981365 2016-11-01 Production Wells Violations Noted On 10/24/ 2016 I conducted a status check of well #2-1642 and found the well to be not in production. I spoke with Enervest Superintendent Tim McConaha concerning the Operators future plans for the well. Mr. McConaha stated he would investigate and communicate back with his findings. While speaking with Mr. McConaha on 11/01/2016, he stated that the well had watered off and Enervest had no plans to place the well back into production. The well is currently incapable of production. Enervest shall submit a written plugging schedule to Inspector Ken Frye at the Ohio Department of Natural Resources, Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685, within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. Enervest will be notified if the plugging schedule is approved or denied.
1936306535 2017-10-13 Plug / Plug Back No Violations The purpose of my inspection was to conduct a status check on well #2-1642 prior to plugging activities. The lease access remained in good repair and drivable. The well location was currently planted in soy beans by a tenant farmer. The well is near overhead electric lines that will restrict positioning of the plugging rig. Correct identification was present. The well was equipped with a plunger lift assembly. No leaks were observed. No violations were observed during this inspection.
2072610228 2017-10-17 Plug / Plug Back No Violations The purpose of my inspection was to witness plug and abandonment activities on well #2-1642. The lease road and location remained stable. Chipco was the plugging contractor. Enervest Southern Ohio Superintendent Tim McConaha and Northern Ohio Superintendent Doug Beighley were on location to direct activities. 1.9375 tubing was placed in the well bore to a depth of 5730 below surface. 30 sacks of Class A cement was pumped through the tubing and displaced with 13.3 barrels of fresh water. I scaled the cement at 14.6 ppg. Circulation was not established. The top of the cement plug was measured at 5315 below surface. After setting plug, the tubing was removed and Chipco prepared to rip the 4.5 casing at 2637. Initial attempts to part the casing were unsuccessful. Operations were suspended for the day until Appalachian Well Surveys could arrived to part the casing. The lined earthen dike constructed to hold well fluids was holding a minimal amount of fluid. No violations were observed during this inspection.
