0301113639 2018-04-26 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1628. RBDMS indicated reported production was current. Access to the well and tank battery remained stable and drivable. Correct identification was present at the well head. The 8.625 well head was above grade and closed to atmosphere. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. Staining consistent with a legacy operation was observed. No leaks were present. A stable earthen dike that was free of fluid was present around the production equipment. Equipment within the dike consisted of one (1) horizontal separator, one (1) 100 bbl production tank and one (1) 1400 gallon poly brine tank. The separator remained in good repair and contained a current 16 day production chart. The production tank remained in good repair and contained correct identification. Valves were closed and plugged. The poly brine tank remained in good repair. The inlet valve was closed. No violations were observed during this inspection.
0374043300 2010-06-02 Production Wells Violations Noted Follow-up inspection revealed that the operator has placed new stone inside the dike and rebuilt the SPCC dike. The follow-up inspection also revealed that the operator has had a clean-up crew use the propane wand and burn off the small pockets of crude oil in the ravine/ditch. The inspection also revealed a very light oil sheen collected at the siphon dam. I will return to the site at a later date to see if any oil has seeped out of the ground anywhere or if anymore has collected at the siphon dam.
0692223699 2010-07-20 Production Wells Violations Noted Follow-up inspection revealed that the tank battery has been reconstructed and a new identification sign has been posted on the oil storage tank. Inspection of the area behind the barn revealed several areas of crude oil leaching up thru the dirt. The larger area that is oil stained is approximately 17 by 20 and there are other areas that are app. 7 by 10 , 8 by 10 , and 4 by 5. The operator needs to finish cleaning up the area by removing any oil saturated soils and disposing of the soils in an approved manner. Note: This is the area that was noted in the initial report as the area that was graded. It appears that the clean-up crew just covered the oil covered vegetation and ground with dirt instead of grading the area and removing the material. I will contact the operator and advise that the clean-up of the spill was inadequate and more remedial action is requried at this time. I will also advise the operator that I will return to the site in the next couple of weeks to see if all the remedial actions have been completed.

1280163495 2010-05-20 Production Wells Violations Noted Rick Beta with North American Petroleum contacted me and advised me that there had been a crude oil / brine release on the Smith # 2 well in Columbiana County due to an oil storage tank that leaked. Upon arrival, I observed that the oil had leaked out of the oil storage tank and and had traveled thru the SPCC dike. The oil storage tank has been removed from the tank battery at this time. The operator has started working on the clean-up process inside and outside the SPCC dike. The oil traveled from the SPCC dike downslope to behind a barn and then into a small ravine. The oil then traveled down the ravine into a ditch and traveled approximately 400 into a wet area. The operator has constructed a siphon dam in this area to contain the oil before it enters a wetland area and a small stream. There is very little oil collected at the siphon dam at this time. While walking down the ravine/ditch toward the siphon dam I found several small (app.2x2) pockets of crude oil scattered throughout the length of the ravine/ditch. I walked back to the tank battery area and observed that the area outside the dike and behind the barn had been graded and the majority of the crude oil appears to have been cleaned up. I contacted Rick Beta and we discussed the small areas of crude oil in the ravine/ditch. There is very limited access for clean-up equipment due to the terrain and location of the ravine/ditch. Decided to have the clean-up crew walk the ravine/ditch with a propane wand and burn the small pockets of crude oil since rain was forcasted. I advised him that I would return to the site to check on the clean-up status after they walked the ravine/ditch with the propane wand and burnt the small pockets of crude oil. Be advised no NOV was issued at this time because the operator was the person that contacted me about the spill.
