0709400730 2017-12-04 Production Wells No Violations The purpose of my inspection was to conduct a status check inspection on well #2-1627. RBDMS indicated production was current. Access to the well remained stable and drivable. The well and tank battery locations were stable. A fenced enclosure was constructed around the well head. The 8.625 casing head was below grade. The annulus was plumbed above grade and open to atmosphere. The 4.5 production casing was equipped with a mandrel well head and electric pump jack. The drive belts were removed. Production valves were in the open position. No leaks were observed. The mandrel head exhibited extensive corrosion and scaling on the fasteners. A fenced enclosure was constructed around the tank battery. An stable earthen dike that was free of fluid was present around the production equipment. Equipment inside the dike consisted of one (1) horizontal separator, one (1) 50 bbl poly production fluid tank and one (1) 1400 gallon poly brine tank. The separator exhibited signs of corrosion. No leaks were observed. A current 7 day chart was in the meter house. The production tank remained in good repair. No cracks or leaks were observed. The inlet was plumbed into the top opening of the tank. The outlet valve was open and plumbed out to the brine tank and a load out line. The poly brine tank remained in good repair with no cracks or leaks. The tank was holding approximately 250 gallons of fluid. The load out line contained a closed and plugged valve. No violations were observed during this inspection.

2105002902 2015-10-27 Production Wells No Violations I was on site to conduct a status check. The tank battery and wellhead are surrounded by a fence with a gate for access. The gate is not locked or secured. No identification was posted. The tank battery consists of one 50 bbl poly production tank, a 35 bbl poly brine tank, and one horizontal separator. The tanks valves are sealed with a plug. There is a meter box with a current seven day chart showing production. A dike is present. The wellhead is equipped with an electric pumpjack. The stuffing box on the pump jack is leaking gas. The access road and site are stable. I called Eric Petroleum at 10:10 AM and told the office about the leak and they said they would have a pumper call me. Pumper Dan called me back at 10:44 AM and I explained to him the stuffing box had a failure and that identification needed to be posted. He said he would get on it. I received a call on 10/28/15 at 2:47 PM from Eric Petroleum letting me know that the repairs to the stuffing box had been made and the identification posted at the tank battery.
