0402727710 | 2003-09-24 | Production Wells | No Violations | The tank battery consists of 2-210 bbl sotrage tanks, 1-100 bbl and 1- 50 bbl salt water storage tanks, 2-separators and a shared dike with permit 1601. |
0989457775 | 2013-09-06 | Production Wells | Violations Noted | I received a call from Mr. David Unkefer, the landowner. He informed me that he had six wells on his property. Three were producing, and three were not. I arranged to meet with Mr. Unkefer, and inspect all six wells. I met with Mr. Unkefer, and we went to all six wells on his property. The Sluss #7 well used to have pumpjack. There is an ID tag at the well. The well is not in operation. All valves at the well head are in an off position. The pump jack has been removed. At the tank pad, which was shared with the Sluss #6, the tank, separator and gas sales meter have all been removed. I researched the production records for the well on RBDMS. There has largely been no production since 2003, with 55 bbls of oil shipped in 2011. There has been no production reported for the well since then. A permit to plug the well must be obtained within 90 days (12/22/13) of the date of this notice, and the well plugged within 180 days (03/22/14) of the date of this notice. |
1198485150 | 2015-05-11 | Production Wells | No Violations | I was on site to conduct a status check. The tank battery was previously shared between the Sluss #6 (#21601) and the Sluss #7 (#21607), but is no longer. Identification was posted on the wellhead. The tank battery consists of one 210 bbl production tank and a horizontal separator. The tanks valves are sealed with a plug and locked. There is one meter box with current 7-day chart showing production. A dike is present and in good repair. The wellhead was equipped with gas powered pumpjack but It has since been removed. All valves at the wellhead are shut. The rods are still present and sticking out of the tubing. The access road and site are stable. The operator will be contacted to inquire about the status of the well. |

1525102904 | 2018-04-30 | Plug / Plug Back | No Violations | The purpose of my inspection was to witness plug and abandonment activities on well #2-1677. Nicks Well Plugging LLC (NWP) was the plugging contractor. I met with Jason Nicholas of NWP and Larry Fetterman, company representative to discuss the objectives of the operation. NWP rigged up on the well head and removed 700 feet of rods from the well. The earthen pit excavated to hold well circulation fluid did not encounter shallow ground water. The pit was lined with an impervious liner and was holding fluid. Additional tubing was placed into the well to a depth of 784 feet. Circulation was established and 20 sacks of Class A cement were pumped and displaced with 2 bbls of freshwater through tubing. Top of the cement plug was 521 feet. Appalachian Well Surveys bond logged the production casing and found the annular cement top at 464. The production casing was shot at 448 and pulled free. Plugging operations were scheduled to finish on 05/01/2018. I will schedule to be present to witness the remainder of the job. |
1708827823 | 2018-05-01 | Plug / Plug Back | No Violations | The purpose of my inspection was to follow-up on compliance notice # 9894577775 and witness plug and abandonment activities on well #2-1607. Nicks Well Plugging (NWP) was the plugging contractor. One joint of 4.5 production casing was removed from the well and circulation established. After establishing circulation, 99 sacks of Class A cement were pumped through the 4.5 production casing. Cement was circulated to surface. The 4.5 production casing was removed from the well and the well topped off with additional cement. The cement held at surface and did not fall back. The lined earthen pit was emptied and backfilled. The 7 surface casing was cut off 48 below grade and a monument plate added prior to backfilling and grading the location. I will schedule a follow-up inspection for final restoration. |
1984314839 | 2016-12-22 | Production Wells | No Violations | The purpose of my inspection was to conduct a Compliance Notice Follow-up on well #2-1607. The owner has obtained a permit to plug the well. The permit expires 05/13/2018. During a meeting with Atlas Field Superintendent Rusty Morrison on 11/02/2016, Mr. Morrison indicated that Atlas intended to plug the well in 2016 but exhausted the budget plugging wells with hydrogen sulfide issues. I field inspected well #2-1607 and found the well location stable. The pump jack had been removed. Production valves remained closed. No audible or visual leaks were observed. I will contact Mr. Morrison concerning a date to schedule plugging prior to the surface owner planting crops. |
