At the time of my inspection well bore conditions remained static. No flow was observed. Nicks Well Plugging arrived on location and tagged the top of cement at 782 below surface. The 2 tubing work string was installed to a depth of 780. Circulation was established and 125 sacks of Class A cement was pumped and displaced with 0.6 bbls of freshwater. The work string was removed and the crew rigged up to pull the 7 surface casing. The casing pulled free and was removed from the well leaving the conductor in place. The work string was reinstalled into the well and 122 sacks of cement was circulated to surface. After removing the work string the well was topped off. NWP planned to excavate the area around the conductor and cut the conductor off a minimum of 30 below the grade of future development. The future surface owner planned to lower the existing grade by ten (10) feet. I will schedule a follow-up inspection to determine the status of final restoration. |

Prior to commencement of operations, the well casing pressure gauge indicated 140 psi. After relieving the well of pressure and flow, Nicks Well Plugging (NWP) installed a 2 tubing work string into the well and pumped the zonal isolation plug. After setting the plug, NWP removed 812.60 of 5.5 OD production casing. The work string was installed back into the well at 680 below surface and operations concluded for the day. I will schedule to be present to witness the remainder of operations. |

At the time of my inspection, all of the surface equipment and materials recovered during plugging operations were removed from the location. I witnessed all fluids removed from the pit and the liner removed on 7/10/2018. The pit area was backfilled and graded. The conductor casing was below grade. A section of sucker rod was in place and flagged to locate the end of the service line. The surface conditions did not require establishing a roadway for ingress/egress. The current surface landowner, Stephen Chenault waived establishing vegetative cover for erosion control and removal of the service line due to the pending sale and redevelopment of the property. The grade and condition of the area adjacent to the area disturbed during plugging operations did not present a concern for substantial damage to an adjoining property, substantial contamination of surface or underground water, or substantial erosion or sedimentation. The location meets the requirements for final restoration. |

The well location was in the backyard of the residence and remained stable with well established vegetation. Identification was present. The 7 inch surface casing was above grade. The 5.5 inch production casing was produced open flow through a flow cross that contained a gauge that indicated 30 psi. The top of the cross contained a master gate valve that was plumbed to an inclined piece of casing utilized as a drip tank. The drip tank contained an outlet for the domestic supply connection and a connection to a pair of steel fluid storage tanks. No leaks were observed. |
