0112931211 2018-03-16 Production Wells No Violations The purpose of my inspection was to document the status of well #2-1558. No direct access to the well was present. The well location remained stable. A locked chain link fence enclosure was present around the well head with mature trees growing through the enclosure. Correct identification was present. The 8.625 well head contained a swage with a valve that was plugged. No leaks were observed. No production equipment was on location.

At the time of my inspection, no direct access was present to the well head. The well was enclosed within a locked chain link enclosure with mature trees growing through the top of the enclosure. Correct identification was present on the fence. The surface casing contained a swage and a closed valve that was plugged. No leaks were observed. I photographed the location at the request of ODNR-DOGRM Attorney Brendan Inscho who was finalizing a Chiefs Order to plug the well.

1320287848 2017-11-30 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1558. The operator has reported zero (0) production in the last two years. Access to the well was off of the township road through woodlands. No direct access to the well head was present. The well location was stable. A locked, chain-link fence enclosure remained around the well. Trees were growing through the roof of the enclosure. The production casing contained a swage with a closed and plugged valve. No leaks were observed. No production equipment was present. No identification was present. I called and spoke with Everflow Eastern field manager George Strawn concerning the status of the well. Mr. Strawn was not familiar with the well and requested time to investigate the well with the office. I will plan to follow up with Mr. Strawn regarding the status of well #2-1558.

1512410013 2017-12-04 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1558. No direct access to the well was present. I made a trail through woodlands to locate the well head. The well location was stable. A locked chain-link fence enclosure was constructed around the well. Trees were growing through the top of the fenced enclosure. No identification was present. The 8.625 well casing was equipped with a swage that was topped with a closed and plugged valve. No leaks were observed. I called and spoke with Everflow Eastern field manager George Strawn concerning the well. Mr. Strawn was unfamiliar with the well and stated that he would discuss the situation with the office and communicate back with the status of well #2-1558.

The well location remained stable. A locked chain link fence enclosure was present around the well head with mature trees growing through the enclosure. Correct identification was present. The 8.625 well head contained a swage with a valve that was closed and plugged. No leaks were observed. No production equipment was on location. The completion record on file indicated total drilling depth was reached on 11/15/1992. ODNR-DOGRM has no documentation that the well was ever completed. The well owner has not reported production from 1992 through 2016. The well owner reported zero (0) production in 2017. The well is currently incapable of production and Everflow Eastern /Partners LP shall submit a written plugging schedule to Inspector Kenneth Frye at the Ohio Department of Natural /Resources, Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685, within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. Everflow Eastern Partners LP will be notified if the plugging schedule is approved or denied.
