0814637996 2015-07-27 Production Wells No Violations I was on site to conduct a status check. Identification was posted on the tank battery. The tank battery consists of one 210 bbl production tank, a 35 bbl poly brine tank, and one horizontal separator. The tanks valves are sealed with a plug. There is a meter box with a current seven day chart showing production. A dike is present and in good repair. The wellhead is equipped with an electric pumpjack and a domestic supply line. The access road and site are stable. No violations were observed.
0824314204 2018-12-27 Production Wells No Violations The purpose of my inspection was to follow-up on compliance notice #924378039. At the time of my inspection, the correct identification was present on the separator door and the production tank body. The replacement production tank was in good repair. Valves were closed and plugged. The drain valve was connected to the poly brine tank. The inlet valve was also closed. The separator contained a current 24 hour production chart. The earthen dike remained in good repair. No further action is necessary.
0898875147 2018-11-02 Production Wells No Violations The purpose of my inspection was to document the status of the well #2-1501. ODNR-DOGRM emergency response coordinator Fred Romeo notified me that a fire involving the production tank had occurred prior to 10/29/2018. At the time of my inspection access to the well and tank battery remained drivable and stable. The road was also utilized by the surface owner for agriculture. The well site remained stable. Correct identification was not present at the well head. The 8.625 well head was below grade. The 4.5 production casing was equipped with an electric pump jack assembly. Production valves were in the closed position. No leaks were observed. Staining consistent with a legacy operation was present. A domestic supply connection was present. A stable earthen dike was present around the production equipment. Equipment on location consisted of one (1) horizontal separator and one (1) 1200 gallon poly brine tank. Correct identification was not present at the tank battery. The separator remained in good repair and contained a current 7 day chart that showed the well was shut in. The brine tank remained in good repair. Valves were closed and plugged. The brine tank did not incur any damage from the fire. The damaged production tank was not on location. The separator outlet to the tank was plugged. I did not observe any free product within the diked area. I did observe a de minimis amount of burn damage to vegetation adjacent to the tank battery. A spring discharge/ surface drainage was adjacent to the East side of the tank battery. I did not witness any evidence of overflow or contamination from the incident leading to the surface water. Following the inspection I spoke with lease operator Don Stonemetz. Mr. Stonemetz reported that a new tank with identification was on order and would be installed within the next week or two. Mr. Stonemetz agree to contact me once the tank was installed to facilitate a follow-up inspection. I will schedule a follo
0924378039 2018-11-29 Production Wells No Violations The purpose of my inspection was to document the status of the production tank battery at well #2-1501. At the time of my inspection the production tank had not been replaced. Identification was not present at the well head or tank battery. I spoke with lease operator Don Stonemetz on 12/10/2018 regarding the status of repairs and to notify him that I was issuing a compliance notice for no identification at either the well or tank battery. Mr. Stonemetz reported that the production tank was due to be installed with identification on or before 12/21/2018. He continued that he would call to report when the tank was installed. I will schedule a follow-up inspection once the repair is completed.
