0605798773 2018-04-12 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1416. RBDMS indicated the reported production was current. Access to the well and tank battery was stable. A locked chain-link fence enclosure was present around the well head. No identification was present. The 8.625 production casing was below grade. The 4.5 production casing was equipped with an electric pump jack. Production valves were in the open position. No leaks were observed. A domestic supply connection was present. A locked chain-link fence enclosure was present around the tank battery. No identification was present. A stable earthen dike that was free of fluid was present around the equipment. Equipment within the dike consisted of one (1) horizontal separator, one (1) 100 bbl production tank and one (1) 1200 gallon poly brine tank. The separator remained in good repair and did not contain a current production chart. The production tank remained in good repair. Valves were closed and plugged. The poly brine tank remained in good repair with no evidence of cracks or leaks. The tank was holding a de minimis amount of fluid. Valves were closed and plugged. Following the inspection I spoke with contract lease operator Jeremy Kohler. Mr. Kohler reported that the company had new signs made and were in the process of installing the signs as conditions permitted. I will schedule a follow-up inspection to document the status of identification.

1493682712 2018-05-14 Production Wells Violations Noted The purpose of my inspection was to document the status of identification at well #2-1416. At the time of my follow-up inspection I did not observe identification at the well or tank battery. Following the inspection I spoke with contract lease operator Jeremy Kohler and inquired about the status of the identification. Mr. Kohler reported that the operator had supplied the identification sign but the sign had not been posted due to the well being shut in and operated intermittently. I communicated to Mr. Kohler that I would be issuing a compliance notice for failing to legibly identify the well and that the identification needed to be posted within 7 days. Mr. Kohler stated that he would have the identification posted later that day. I will schedule a follow-up inspection to document the status of identification.
