0125190704 2016-10-17 Production Wells No Violations The purpose for my inspection was to conduct a status check on well #2-1400. RBDMS indicates the operator is current with reporting production. The lease road to the well and tank battery is stable and drivable. The well head was missing identification. 8.625 surface casing head is above grade and contains a plug in one side port and a valve open to atmosphere in the other port. 4.5 production casing is equipped with a plunger lift assembly. Production valves were in the closed position. A 4.5 production casing pressure gauge indicated 360 psi. No audible or visual leaks were detected. Production equipment is enclosed by an earthen dike that is stable and sufficient for containment. Equipment within the dike consisted of one (1) horizontal separator, one (1) 210 bbl production tank and one (1) 35 bbl poly brine tank. The separator remained in good repair. No audible or visual leaks were detected. The production chart in the meter house was expired and indicated an extended shut in period. The production tank contained correct and legible identification conspicuously displayed on the tank body. No leaks were detected on the tank. The load line valve is closed and plugged. The brine drain valve is closed and plumbed to the brine tank. The hatch lid is closed. The poly brine tank is in good repair. No cracks were observed. The inlet valve was in the closed position. A flow line leading to the tank battery is on the surface. The flow line direction does not correspond with the location of the well head. I spoke with Mr. Bob Foltz to replace the identification at the well head and determine the flow line source leading into the tank battery.

1728019798 2018-07-03 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1400. RBDMS indicated reported production was current. Access to the well and tank battery remained stable and drivable. At the time of inspection NEXUS pipeline activity was present. Correct identification was at the well head. The well was equipped with a plunger lift assembly. Production valves were open. No leaks were observed. A stable earthen dike was present around the production equipment. Equipment on location consisted of one (1) horizontal separator, one (1) 210 bbl production tank and one (1) 1400 gallon poly brine tank. The separator remained in good repair and contained a current 16 day chart. The production tank remained in good repair and contained correct identification. Valves were closed and plugged. The poly brine tank had a crack in the top that did not impair fluid holding. The tank was holding a de minimis volume of fluid. The inlet valve was closed. No violations were observed during this inspection.
