0335752444 2016-08-23 Production Wells No Violations The purpose of my inspection was to conduct a status check of the Elzie Smith #1 (#2--1370). An earthen dike constructed around the production equipment is stable and sufficient for containment. Within the dike, the production equipment consists of two (2) horizontal separators, one (1) 140 bbl production tank, one (1) 210 bbl production tank and one (1) 50 bbl brine tank. Production equipment specific to Smith #1 included a horizontal separator, 210 bbl production tank and a 50 bbl common brine tank. The horizontal separator contained a current production chart. Crude oil was observed leaking from the inlet plumbing underneath the separator. The oil was not fresh, pooling or migrating. The production tank contained legible, correct and conspicuous identification. The load line valve was closed, locked and plugged. The brine drain valve was closed, locked and plumbed to the brine tank. No leaks were observed on the tank body. The hatch lid was closed. I will contact Envervest Field Superintendent Tim McConaha to determine responsibility and facilitate the cleanup and repair of the brine drain line in addition to repair of the leaks associated with the horizontal separator.

1056717374 2003-06-26 Production Wells Violations Noted A stuffing box leak has caused oil contamination around the wellhead. The oil is fresh oil. Peat sorb has been placed on the contamination. Remove and properly dispose of the contamination. The size of the area is approximately 3 X 8.
