0246357258 | 2006-09-20 | Production Wells | No Violations | Follow-up inspection revealed that most the oil that was on the pond has been cleaned-up . They have flushed the field tile with fresh water and there is no oil coming out of the field tile at this time . The operator is doing a good job with the clean-up operations at this time . I will return to the site at another date to check on the clean-up progress . |
0306159746 | 2015-07-27 | Production Wells | No Violations | I was on site to conduct a status check. Identification was posted at the tank battery. The tank battery consists of one 100 bbl production tank, a concrete brine vault, and one horizontal separator. The tanks valves are sealed with a plug and locked. There is a meter box with a current seven day chart showing production. A dike is present. The wellhead is equipped with an electric pumpjack. A domestic supply line is coming off the wellhead. The access road and site are stable. |
0419757007 | 2006-09-18 | Production Wells | No Violations | FOLLOW UP TO NOV # 1704657288 Terry Whitehair ( Atlas America ) contacted me and advised that they had an oil storage tank leak and app. 4 bbls of crude oil leaked from the tank and thru the bottom of the dike into a field tile , that flows into a farm pond . The oil made its way from the pond into the road ditch along Lowmiller Rd. .When I arrived they already had the oil removed from the road ditch and was working on cleaning the oil off the pond surface . They had the oil storage tank pulled up on the dike to inspect the bottom of the tank . I advised the crew to keep working on the clean-up and to construct a siphon dam at the pond overflow to prevent the escape of anymore oil from the pond into the road ditch . I advised that I would return to the site in a couple of days to check the progress of the clean-up operations . |
0567575823 | 2017-07-27 | Production Wells | No Violations | The purpose of my inspection was to conduct a status check on well #2-1092. RBDMS indicated production reporting was current. The road, well and tank battery locations were stable. The 4.5 production casing was equipped with an electric pump jack. Identification was present. No leaks were detected. Some residual staining consistent with legacy operations was present on the tubing head. A stable earthen dike that was free of fluid was present around the production equipment. Equipment inside the dike consisted of one (1) horizontal separator, one (1) 100 bbl production tank and one (1) concrete brine vault. The separator remained in good repair and contained a current 7 day production chart. The production tank remained in good repair with correct identification. Valves were closed, locked and plugged. The vault remained in good repair above grade. The vault was holding fluid with free board. No violations were observed during this inspection. |

1161893336 | 2006-10-25 | Production Wells | No Violations | Follow-up inspection revealed that the oil storage tank has been removed . Inspection of the area revealed a small amount of crude oil on the pond near the end of the field tile . They have installed an oil boom at the end of the field tile to contain any crude oil that comes out the filed tile . The rest of the farm pond appears to be free of any crude oil at this time . The siphon dam at the pond overflow is still intact and functioning . I spoke with Andy Smith ( pumper ) and he requested an extension . I advised that I would extend the remedial date to Nov. 10th , 2006 . |

1402084144 | 2012-10-01 | Annular Disposal | No Violations | This was a routine inspection to determine if annular disposal is taking place at the well. I found no annular disposal connection at the well head. At the tank battery, there is a concrete brine tank with a piping connection to the 100 barrel storage tank. Other equipment at the tank pad include a horizontal separator with a meter box. A chart in the meter box indicates that the well is in production. The well has a pump jack. |
