0718311044 2018-08-27 Production Wells Violations Noted The purpose of my inspection was to document the status of the leaking tubing head packing assembly on well #2-1025. At the time of my follow-up inspection gas was witnessed leaking from the tubing head assembly. On 9/5/2018 I notified Enervest Field Superintendent Joe Teppert that the well was insufficiently equipped to prevent the escape of gas. Mr. Teppert reported that he would have a contractor scheduled to investigate the leak and make any necessary repairs within two weeks. I will schedule a follow-up inspection after 9/19/2018 to document the status of the well.
1536146650 2018-08-16 Production Wells No Violations The purpose of my inspection was to conduct a status check inspection on well #2-1025. RBDMS indicated reported production was current. Access to the well remained stable and drivable. The 8.625 well head was plumbed above grade and closed to atmosphere. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. Staining consistent with a legacy operation was present. At the time of my inspection I witnessed gas leaking from the packing on the tubing head assembly. A stable earthen dike that was free of fluid was present around the production tanks. A horizontal separator was on located outside of the dike. The separator remained in good repair and contained a current 16 day chart. One (1) 210 bbl production tank and one (1) concrete brine vault were present inside of the dike. The production tank remained in good repair and contained correct identification. Valves were closed and plugged. The brine vault remained in good repair above grade. The vault was holding fluid with freeboard. Following the inspection I spoke with lease operator Don Pahovey to notify him of the leak observed at the tubing head. Mr. Pahovey stated he would investigate the leak and make any necessary repairs. I will schedule a follow-up inspection to document the status of repairs.
1670917143 2018-10-15 Production Wells No Violations The purpose of my inspection was to follow-up on compliance notice #718311044. At the time of my follow-up inspection the gas leak at the well head was repaired. No further action is necessary.
