0849996267 2012-08-29 Production Wells No Violations I received a call from John Chenevey on 07/26/12 regarding the well being idle. Mr. Chenevey indicated that the well hadnt produced in years, but some oil that had been in the tank was sold in 2011. Upon inspection, I found that the well was not producing. The tank battery was overgrown with vegetation. There is no separator. The lease road is overgrown and rutted. The pump jack was overgrown with vegetation. The electric box breaker was off, and everything inside appeared to be corroding.

There is neither a separator nor meter. The lease road is overgrown and rutted. The pump jack was overgrown with vegetation, and there were no signs of operation. There was a tag on the piping at the well head indicating that solvent had been put into the well by B&E Well Treating in November 2012. The well must be put into production within 30 days (02/02/13) of the date of this notice, or plugged within 90 days (04/03/13) of the date of this notice.
1385399085 2013-01-09 Administrative Inspection No Violations I spoke with Kathy at Quality Oil & Gas. She referred me to Brian, the pumper for the well. I called Brian and discussed the status of the well. He indicated that they could not sell any gas off the well, but did operate the pump jack about once a month to produce oil. Pumping once a month helped keep the electric bill down. He indicated that he had approximately 7 to 8 feet of oil in the oil storage tank. Since the company is producing the well, the Compliance Notice requesting produce the well or plug or plug the well is closed.
1571027072 2017-03-16 Production Wells No Violations The purpose of my inspection was to respond to a reported spill at well #2-1004. I received a phone call from Scott Miller who stated he was substituting for Dave Widdick while on vacation. Mr. Miller continued that on the morning of 3/16/2017 he had discovered a well head failure caused crude oil to leak onto and migrate adjacent to and down grade from the well location. Due to a recent snow event, the entire area was covered with snow and Mr. Miller was in the process of determining the volume and extent of the release. I confirmed with Mr. Miller that the pumping unit had been shut down to prevent further contamination. Mr. Miller reported that he was unsure of the exact volume of the release, but estimated the volume around 5 bbls. He continued that he had equipment arriving on site to begin containing and cleaning up the impacted soil. I communicated to Mr. Miller that I would be on location as soon as possible. Following the call with Mr. Miller, I spoke with ODNR-DOGRM Emergency Response Coordinator Fred Romeo. ERC Romeo reported to begin grading the contaminated area and store the material in an impervious containment (either lined roll off boxes or lined pit) until disposal can be arranged. I arrived on location to find Mr. Miller grading off the top layer of impacted soil and stockpiling it near the well head. Mr. Miller delineated the extent of the spill and determined that the spill had been contained to the Unit and no production fluid had migrated off of the property or into a surface water body. I walked the designated area and confirmed the delineated area of contamination. Mr. Miller constructed an earthen dike near the lease entrance road and lined it with an impervious pit liner to contain the stockpiled material until a manifest could be obtained. I will follow up with Dave Widdick to confirm the material has been removed from the location and disturbed areas regraded and stabilized.
