0432339815 2016-10-26 Production Wells No Violations The purpose of my inspection was to conduct a status check of the repairs performed on well # 2-0980 due to a failed buried concrete brine vault. An earthen dike is constructed around the production tank and is poly brine tank that is sufficient for containment. The dike walls have been stabilized with seed and mulch. Vegetation is beginning to become established. The poly brine tank is in good repair and plumbed to the production tank. The inlet valve is closed. No further action is necessary.
0462846964 2016-09-01 Production Wells No Violations On September 1, 2016 I was on location to witness the excavation of the failed concrete brine vault from the Bandy #1 (API # 2-0980) tank battery. Mark Schubert with Appalachian Energy was contracted by G&O Resources to perform the vault removal and excavation of contaminated soil. Prior to excavation, the vault was emptied of all fluids and steam cleaned to prevent any additional release of production fluid during the removal process. Following vault removal, I observed the area adjacent to the vault to be composed mostly of clay soil that contained the escaped fluids to the area immediate to the vault and within the diked containment area. I directed Mr. Schubert to remove impacted materials until reaching an area within the dike that had not been impacted. Impacted materials were loaded onto a lined truck and transported to an approved waste facility. There was not enough clean fill on location to completely fill the excavated area and rebuild the dike walls. Mr. Schubert planned to have additional clean fill delivered to properly fill the former vault area and rebuild the dike walls. Per G&O Resources, once the containment area was restored, a poly brine tank would be set and plumbed to the production tank prior to placing well # 2-0980 back into production. I will conduct a follow up inspection once the work is completed and the well is back in production.
0782156635 2017-10-18 Production Wells No Violations The purpose of my inspection was to document the status of production tank replacement on well #2-0980. I witnessed Sutton Supply hauling away the former 210 bbl production tank. On location, I observed the earthen dike constructed around the 210 bbl production tank and 1250 gallon poly brine tank. The tank was free of fluid. The new 210 bbl production tank was plumbed to the horizontal separator and poly brine tank. The load line valve was closed and plugged. The brine drain valve was closed, locked and plumbed to the brine tank. The drain line contained a sight glass. The poly brine tank remained in good repair and was holding approximately 1000 gallons of fluid. The separator remained in good repair. No leaks were observed. I also inspected the well location and found the identification at the well to be updated with the new owner and emergency contact number. No leaks were observed. While on location I spoke with Alliance Petroleum Corporation Superintendent Matt Postelthwaite. Mr. Postlethwaite reported the tank replacement was performed for preventative maintenance purposes. No violations were observed during this inspection.
1361813299 2016-09-14 Production Wells No Violations I spoke with Mark Schubert of Appalachian Energy to determine the repair status of the tank battery for well # 2-0980. Mr. Schubert communicated that the production pad and earthen dike were rebuilt. Mr. Schubert still needed to plumb in the poly brine tank and seed and mulch the dike walls for stabilization. I will conduct a follow up inspection once the work is completed and the well is back in production.
1449663263 2015-11-28 Production Wells No Violations I was on site to conduct an inspection regarding a report about potential oil in the stream adjacent to the wellhead. The wellhead had a pump jack on it. There was a domestic line. The area around the wellhead was clean and the valves were open. There were no visible signs of any leaks or odors at or around the wellhead during the time of this inspection. No staining was observed at or near the wellhead. Upon inspection of the stream, it was observed there was no oil, but organic decomposition that had accumulated against fallen woody debris within the stream. No staining was observed in the stream or adjacent to the bank of the stream, during this inspection. I also conducted an inspection of the tank battery. The tank battery consisted of a 100 BBL production tank, buried concrete brine vault, and mechanical separator. The following adjacent wells were also inspected: 21821, 21822, 21810, 20981, 21039, 21033, and 21034. None of these wells had any issues related to the above request and no violations were found.
1449681242 2015-12-09 Production Wells No Violations I was on site to conduct a status check. Identification was posted at the tank battery and wellhead. The tank battery consists of one 210 bbl production tank, a brine vault, and one horizontal separator. The tanks valves are sealed with a plug. There is a meter box with a current 24-hour chart. A dike is present. The wellhead is equipped with an electric pumpjack. A domestic supply line is coming off the wellhead. The access road and site are stable.
