0447143332 2017-07-27 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-0935. RBDMS indicated production reporting was current. The road, well and tank battery locations were stable. The 4.5 production casing was equipped with an electric pump jack. Identification was present. No leaks were detected. A domestic supply connection was present. A stable earthen dike that was free of fluid was present around the production equipment. Equipment inside the dike consisted of one (1) horizontal separator, one (1) 210 bbl production tank and one (1) concrete brine vault. The separator remained in good repair and contained a current 7 day production chart. The production tank body remained in good repair with correct identification. Valves were closed, locked and plugged. Staining consistent with a legacy operation was present at the plugged heater tubes. No fresh or live fluid was observed. The vault remained in good repair above grade. Some staining consistent with legacy operations was present on the side of the vault and ground next to the tank was present. The fluid was not fresh or live. The vault was holding a de minimis volume of fluid. I spoke with Atlas field superintendent Rusty Morrison while on location concerning the staining and the need to monitor or maintenance the equipment as necessary to prevent a release. No violations were observed during this inspection.

1319232002 2005-11-21 Annular Disposal No Violations The AD is not in use or hooked up.
1354816587 2012-10-01 Annular Disposal No Violations This was a routine inspection to determine if annular disposal is taking place at the well. I found no annular disposal connection at the well head. At the tank battery, there is a concrete brine tank with a piping connection to the 210 barrel storage tank. The valve from the storage tank to the brine tank was locked. Other equipment at the tank pad include a horizontal separator, with meter box. The well has a pumpjack. A chart in a meter box on the separator indicates the well is in production.
1825064402 2006-11-15 Production Wells No Violations Routine inspection revealed no violations at this time .
2071685550 2015-07-27 Production Wells No Violations I was on site to conduct a status check. Identification was posted at the tank battery. The tank battery consists of one 210 bbl production tank, a concrete brine vault, and one horizontal separator. The tanks valves are sealed with a plug and locked. There is a meter box with a current seven day chart showing production. A dike is present. The wellhead is equipped with an electric pumpjack. A domestic supply line is coming off the separator. The access road and site are stable.
