0035010067 2018-04-09 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-0883. RBDMS indicated reported production was not current. Access to the well and tank battery remained stable and drivable. Correct identification was present. The 8.625 well head was above grade and closed to atmosphere. The 4.5 production casing was equipped with an electric pump jack. Production valves were in the open position. A domestic supply connection was present. I witnessed the stuffing box head was leaking production fluid onto the well head and collecting on the ground. The fluid was not live or mobile. A stable earthen dike that was free of fluid was present around the production equipment. Equipment within the dike consisted of one (1) horizontal separator, one (1) 210 bbl production tank and one (1) concrete brine vault. I observed the pressure relief valve had failed recently and discharged production fluid onto the ground beneath the separator. The operator had spread sawdust underneath the vessel to absorb the material. The relief valve was replaced with a plug. The separator contained a current production chart. The production tank remained in good repair and contained correct identification. Valves were closed and plugged. The vault was in good repair above grade and was holding fluid with freeboard. Following the inspection I spoke with production foreman Chris Belch regarding the leak. Mr. Belch stated he had replaced the former operator and would oversee repairs on 4/11/2018. I will schedule a follow-up inspection to document the repairs.
0587323921 2018-04-20 Production Wells No Violations The purpose of my inspection was to document the status of well head repairs on well #2-0883. At the time of my inspection the operator had cleaned up excess production fluid off of the well head assembly, cleaned the ground area around the well head and spread saw dust to absorb any additional material. I will schedule a follow-up inspection at a later date to document the status of repairs. No further action is required at this time.

1574956747 2015-07-13 Production Wells No Violations I was on site to conduct a status check. Identification was posted at the tank battery. The tank battery consists of a 210 bbl production tank, a concrete brine vault, and a horizontal separator. The tanks valves are plugged and locked. There is a meterbox with a current 7 day chart showing production. A dike is present and in good repair. The wellhead is equipped with an electric pumpjack and a domestic supply line. The access road and site are stable. No violations observed.
