1920244047 2018-07-23 Field Inspected, Well Not Found No Violations The purpose of my inspection was due to a request from the ODNR DOGRM enforcement section. At the time of my inspection, ODNR-DOGRM Inspector Jonnie Patrick had reported the well was plugged per inspection on 11/21/2005. A review of RBDMS did not indicate a plug permit or report submitted for well #2-0644. While on location I reviewed the permit and plat locations for #2-0641 and #2-0644 and determined that well #2-0641 was misidentified by the current operator as Kitzmiller #2 (#2-0644). The well location for #2-0644 was within a mature corn crop that would prevent locating the former location at the time of inspection. A former operator had received a permit to plug and abandon well #2-0641. During the inspection I called and spoke with current lease operator Don Pahovey with Enervest. Mr. Pahovey was not aware that the well was misidentified and suggested I speak with former lease operator Steve Pegg. I spoke with Mr. Pegg, who operated well #2-0641 while owned by Northcoast Exco. Mr. Pegg stated that the well was identified as the Kitzmiller #2 while he operated the lease, but believed the well should have been identified as the #1. He also stated that he never operated any other well on the lease. I also spoke with Enervest field superintendent Joe Teppert concerning the situation. Mr. Teppert was also unaware of a second plugged well on the lease and was only familiar with well #2-0641 that was identified as the Kitzmiller #2. Mr. Teppert continued that he would contact Enervest land agent Troy Valasek regarding the situation. I will refer the case to the ODNR-DOGRM enforcement section to work with the current operator, Enervest, to correct identification at the well and tank battery for well #2-0641.
