0658768098 2018-06-04 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-0622. RBDMS indicated reported production was current. Access to the well was by foot through an agricultural field. The 8.625 well head was at grade. The 4.5 production casing was equipped with tubing. Production valves were in the closed position. The plunger catcher was removed from the tubing head. The tubing above the head was not vertical. I witnessed a leak from the nipple on tubing head out to the domestic supply connection. The connection was not positioned square in the head. Production equipment consisted of one (1) pressure separator and two (2) steel production tanks. The separator remained in good repair. The meter box contained an expired chart. The dike around the equipment was worn. While on location I spoke with Enervest field superintendent Joe Teppert. I communicated my findings and inquired about the responsibility for the leak. Mr. Teppert stated that the domestic connection from the wellhead was the responsibility of the domestic user. He continued that he would schedule Scott Miller to make necessary repairs as early as 6/05/2018. I will schedule a follow-up inspection to document the status of repairs.
