I had previously spoke with Mark Van Tyne at Pin Oak Energy on 6.23.2017 concerning the surface owner placing the debris on the lease road and causing a safety hazard. Mr. Van Tyne stated that Pin Oak had contacted surface owner Edwin Hunter concerning the debris and that Mr. Hunter was to contact Inspector Kenneth Frye regarding the removal of the debris by 7.7.2017. While on location I called and spoke with Mr. Van Tyne to communicate that I had not ben contacted by Mr. Hunter and that the concrete remained in place. Mr. Van Tyne stated that he would make contact with Mr. Hunter again in order to have the debris removed. I will schedule a call with Mr. Van Tyne to determine the status of debris removal. |
0488239612 | 2017-08-07 | Production Wells | No Violations | The purpose of my inspection was to meet with surface resident Edwin Hunter and lease operator Jessie White from Pettigrew Pumping to discuss removal of concrete debris that was obstructing access to the well head. Mr. White had vehicle trouble and was unable to attend the meeting. Mr. Hunter explained the concrete was placed there to prevent vandalism and damage to his property, not to prevent access to the well. Mr. Hunter agreed to accommodate Pin Oak Energy in accessing the well for service work and routine operations. Mr. Hunter also brought to my attention the flow line from the well to tank battery was never buried. Current vegetation prevented documenting this claim. While on location I called and spoke with Steve Yurick of Pin Oak Energy to discuss the conversation with Mr. Hunter and request burial of the flow line and removal of a failing brine tank. Mr. Hunter offered to assist with clearing of brush and vegetation prior to flow line burial. I will schedule a follow up inspection to document the status of the flow line burial and tank removal. |

A review of RBDMS indicates production has not been reported for 2016. Lease access was stable and drivable. The surface owner has piled broken concrete on the access road to the well head. An abandoned mobile home trailer was stationed adjacent to the well head. The well location was stable with well-established vegetation and an apron of slag. The identification sign needed the new owner identified. All other information was correct. The 8.625 well head was below grade. The 4.5 production casing was equipped with an electric pump jack. Production valves were in the open position. The condition of the polishing rod indicated that the pumping unit had not been operated recently. No audible or visual leaks were detected. Minor staining consistent with a legacy operation was present around the well head. The fluid was not fresh or live. A domestic supply connection was present. The tank battery was located adjacent to a wooded area. The tank battery location was stable. Any earthen dike was eroded away. The surface owner had abandoned vehicles and other refuse placed near and inside the tank battery. The surface owner had also been open burning refuse within one hundred (100) feet of the tank battery. Production equipment for well #2-0599 consisted of one (1) horizontal separator, one (1) 210 bbl production tank and one (1) 100 bbl brine tank. Inlet flow valves to the separator were in the open position. No audible or visual leaks were observed. Correct and legible identification was present on the 210 bbl production tank. Moderate exterior corrosion was present on the body of the tank. No holes or leaks were observed. The load line valve was closed and plugged. The drain line valve was closed and plugged. The tank was holding minimal fluid. The hatch was closed. I observed the heater tube and the base of the brine tank leaking production fluid. The fluid was not |
1221247158 | 2017-08-07 | Production Wells | No Violations | The purpose of my inspection was to meet with surface resident Edwin Hunter and Pin Oak Energy Partners LLC (Pin Oak to discuss the removal of concrete debris Mr. Hunter had placed on or near the lease access road to the well head and other debris activities near the tank battery that were a cause for concern. Mr. Hunter had no objection to removing the concrete to facilitate access to the well and to cease open burning in close proximity to the tank battery. Mr. Hunter did request that the lease access road be stabilized to prevent erosion or rutting from traffic to the well. Mr. Hunter also brought to my attention that the flow line between the well head and tank battery remained above ground. Thick vegetation at the time of inspection prevented confirmation of this statement. While on location I called and spoke with Derek Bryant of Pettigrew Pumping to determine the whereabouts of the operator. Mr. Bryant stated that the pumper had vehicle trouble and would not be able to make it. While on location I called and spoke with Steve Yurick at Pin Oak Energy to discuss the meeting with Mr. Hunter and address access and road maintenance concerns along with flowline burial. I will plan to follow up to ensure the work gets completed. |

A stable earthen dike that was free of fluid was present around the production equipment. Equipment inside of the dike consisted of one (1) vertical separator and one (1) 100 bbl production tank. The former tanks were removed. The replacement tank was in good repair and contained correct identification. Valves were closed and plugged. The flow line was still above grade in one section from the well head. I called and spoke with Pin Oak Energy field manager Ted Lawer concerning the flow line. Mr. Lawer stated he would schedule Ted Cutright to bury the line. I will schedule a follow up inspection after the flow line is buried. |

Mr. Lawer stated that Pin Oak planned to brush the flow line right of way and bury the flow line from the well to the tank battery. He continued that Pin Oak would improve/construct an earthen dike around the equipment and would be removing the leaking brine tank. Access to the well head was stable and drivable. The surface owner had moved concrete debris that prevented vehicle access to the well. Identification remained in place at the well head. The 4.5 production casing was equipped with an electric pump jack. Production valves were in the open position. A domestic supply connection was present and active. No leaks were observed. Mature vegetation remained in place and obscuring the flow line. No improvement activity had occurred at the tank battery. No earthen dike was present around the equipment. Equipment on location consisted of one (1) vertical separator, one (1) 210 bbl production tank and one (1) 100 bbl brine tank. The separator was not utilized. The production tank contained identification and remained in good repair. Valves were closed and plugged. The brine tank heater tube remained unplugged and had leaked a small amount of fluid to the ground. The fluid was not fresh or live. I will contact Mr. Lawer to determine the status of scheduled repairs. |
