1154444707 2016-11-08 Production Wells No Violations The purpose for my inspection was to conduct a status check on well # 2-0594. A review of RBDMS indicates the Operator is current with reporting production. Access to the well head and tank battery is stable and drivable. The well location is on the Sleepy Hollow Golf Course and is stable. The 8.625 surface casing head is above grade and fitted with two plugs in the side ports. The 4.5 production casing is equipped with a electric pump jack assembly. Extensive scaling was observed on the 4.5 production casing landing joint beneath the stuffing box. No audible or visual leaks were detected. A domestic supply connection is present. An earthen dike that is stable and sufficient for containment, Equipment within the dike consists of one (1) vertical separator and one (1) 100 bbl production tank. Minimum spacing requirements are met. The separator is in good repair with no leaks. There is no production chart in the meter house. Correct and legible identification is conspicuosly displayed on the tank body. The tank body remains in good repair with no leaks. The load line valve is closed and plugged. The brine drain line valve is closed and plumbed out to a load line for hauling. Production fluid was observed dripping from threaded connection. The hatch lid was closed. While on location, I called and left a voicemail message with Rick Huffman of Eric Petroleum concerning having the valve repaired. Mr. Huffman returned my call and stated that he had a vaccuum truck scheulded to empty the production tank tomorrow morning followed by replacement and or repair of the leaking connection.
1726734219 2016-11-16 Production Wells No Violations I was on location to confirm the replacement of a leaking production tank valve observed during an inspection on 11/08/2016 (Inspection # 1154444707). A new valve was plumbed into the body of the production tank. The valve was in the closed position and contained a plug. No further action is necessary.
