0709029866 2017-03-09 Storage Well No Violations Per TransCanada well tender Russ Flick, well #2-0515 was plugged in 2012. I will re-inspect the well to determine final restoration status.
1304459242 2013-12-10 Storage Well No Violations I inspected Columbia Gas Transmission gas well # 4494 today. The well is located in Center Township, Columbiana County, Ohio. The well was plugged in 2012. There is a plug permit on file. I was in contact with Keith Crowell a supervisor for Columbia Gas. He is going to get a copy of the paperwork to Inspector Rick Anderson.
1486339982 2018-06-26 Final Restoration No Violations The purpose of my inspection was to document the status of final restoration on well #2-0515. At the time of my inspection any excavated pits had been backfilled and the location regraded to local contour. Vegetation was well-established. No signs of erosion were observed. No debris was on location. While on location I met Columbia Gas storage field operator Russ Flick to assist in locating the former well head location. Mr. Flick indicated that the well head was located under a pile of aggregate stored on location by the surface landowner. Mr. Flick reported that the former well was utilized as an observation well and had no associated lead line. The access road off of the surface owners driveway remained in place. At the time of inspection, Mr. Flick contacted the land department to locate a landowner waiver. Mr. Flick and I met with surface landowner John Burton who stated that he had requested the access road be left in place, but did not recall signing a Form 5 waiver. Mr. Flick communicated that he would have the land department send Form 5 to Mr. Burton for final restoration. I will schedule a follow-up inspection to document final restoration upon receipt of Form 5.
1965074631 2018-09-12 Final Restoration No Violations The purpose of my inspection was to document the status of final restoration. At the time of my inspection the former well location remained stable. The former access road remained in place and the surface landowner was using the site to store aggregate for property maintenance. I have received a signed Form 5 waiver for road restoration work from TransCanada/ Columbia Gas land agent Bob Clark signed and initialed by the surface landowner, John Burton. Final restoration is passed for well #2-0515.
