0286266969 2017-02-21 Storage Well No Violations The purpose of my inspection was to conduct a status check on well #2-0481. The lease road was stable and in good repair. The well location was within an agricultural field that was stable. A pad of crushed gravel was laid around the well head out to the buried main line. The area West of the well location had been excavated. The disturbed area was graded to local contour and stabilized with straw mulch. Vegetation was beginning to become established. Correct and legible identification was posted at the well head. The well head was fitted with risers in both side ports that were plumbed above grade and were fitted with closed and plugged valves. The well head was equipped with a master gate valve that was plumbed out to a side gate valve. The side gate valve was plumbed out to a 4.0 flow line that lead to a vertical drip. The vertical drip outlet was plumbed to the main line. Test valves were closed and plugged. No audible or visual leaks were detected. No violations were observed during this inspection.
1459173468 2016-03-24 Drill / Deepen / Reopen No Violations I was notified by Dave Reynolds of Columbia Gas that they were in the process of reworking this well. The are currently drilling below the 5.5 casing so that they can set and cement 3.5 casing to surface. The cement job for the 3.5 casing will be on 3/25/2016. The company does not have a permit to deepen the well. I notified Regional Manager Tom Hill about the situation.

1782550589 2015-04-21 Storage Well No Violations I conducted a status check on this well. The well is equipped with a master gate valve that is plumbed up to a side gate valve. The side gate valve is plumbed into a 4.00 lead line that goes into the main line. The well is properly identified with a clear, legibly marked well identification sign. The lease road is in stable condition and drivable. No violations found.

1863842152 2014-11-05 Storage Well No Violations I inspected Columbia Gas storage well #4465. The well is equipped with a master gate valve and a side gate valve. There was no storage tank associated with the well. The well is properly identified with a clear, legibly marked well identification sign mounted on the storage tank. The lease road is stable. No violations found.
2104594477 2016-03-25 Casing String Witnessed No Violations I was on location for the cementing of the production string. The service company performing the work over on the storage well is Hagan Well Service. C&J Energy Services are the cement company on location. The 4 3/4 hole was drilled below the 5.5 casing to 998. The 3.5 production casing was set to 995. C&J rigged up, broke circulation, and pumped 50 sacks of lead cement followed by 25 sacks of tail cement. The company scaled the cement at 15.6 ppg. The plug was displaced with 8.5 BBLs of water. The pit was in stable condition and the liner was in good repair. The pit was surrounded by orange safety fence. The location is in stable condition.
2134552064 2013-12-10 Storage Well No Violations I inspected Columbia Gas Transmission gas well # 4465 today. The well is located in Fairfield Township, Columbiana County, Ohio. The well is equipped with a master gate valve that is plumbed up to a side gate valve. The side gate valve is plumbed into a 4 lead line that goes into the main line. The annulus valve is plumbed up and is closed. There is no tank on location. The well is properly identified with a clear, legible identification sign. The lease road is in stable condition. No violations found.
