0340222975 2017-08-21 Plug / Plug Back No Violations The purpose of my inspection was to witness the setting of the bottom isolation plug in well #2-0466. I observed Owner Mr. Robert Foltz remove a final bailer of fluid that contained approximately 4 gallons. No gas was observed flowing from the well. R.W. Sidley placed approximately 1.25 yards of 9 sack grout into the well bore. Mr. Foltz planned to tag the cement top on 8.22.2017 and proceed with plugging operations.
1308830835 2018-06-12 Final Restoration No Violations The purpose of my inspection was to determine the status of final restoration on well #2-0466. At the time of my inspection, the working pit was backfilled and the location graded to local contour. The location was within a woodland. Vegetation was established. No signs of erosion or sedimentation were observed. All equipment and debris utilized in plug and abandonment was removed. The access road was reclaimed. The location has passed the requirements for final restoration.
1503662831 2017-08-24 Plug / Plug Back No Violations The purpose of my inspection was to witness the setting of the top plug in well #2-0466. Owner Bob Foltz pulled 585 of 5.5 intermediate/production casing from the well. Mr. Foltz attempted to pull the 7 surface casing from the well and was only able to recover one joint when the casing string parted. I arrived on location after the casing was removed and counted twenty-one (21) joints of 5.5 casing and one (1) joint of 7 casing. Fresh water fluid level was tagged at 150 below grade. A tremie tube line was placed in the hole to a depth of 550. Total depth of the hole was 572 below grade. R.W. Sidley delivered 4.5 yards of 9 sack grout. The grout was poured into the annular space between casing and tremie tube. I witnessed flow from the tremie tube. When the tremie stopped flowing, Mr. Foltz began to pull the tubing from the well-bore. All joints of tubing pulled dry. After removing the tremie tube, the remaining grout was placed into the well-bore to a depth of 150 below grade. Mr. Foltz planned to top off the surface plug when setting the next plug. I will schedule to be on location when the top plug is topped off.
1591183542 2017-07-18 Plug / Plug Back No Violations I was on location with Inspector Ken Frye to cement the bottom plug on this well. Sidley Concrete brought 1.5 yard of 9 sack grout slurry and it was feed down the well bore at a slow rate. Foltz ran 819 15 feet from TD of 1.5 tubing to use as a tremie tube. The well depth was 834. There was a good flow of fluid coming out of the tremie tube. Will check top of cement 7/20/2017
2002282574 2017-08-17 Plug / Plug Back No Violations The purpose of my inspection was to document the status of plugging activities on well #2-0466. Owner Bob Foltz was on location and disassembling the well head to begin removing tubing. An earthen pit was dug and lined with an impervious liner. The location was stable. No violations were observed during this inspection.
2146818785 2015-11-18 Production Wells No Violations My inspection found the well to be equipped with 8 1/4 casing, 6 5/8 casing, and 5 3/16 casing. There is 2 tubing and rods in the well. The pump jack has been removed from the well. The annulus is open to the atmosphere. The tank battery has been removed by the well owner. There is no known production ever recorded for the well. The well owner is currently working to plug other berea wells on the property.
