0257913027 2018-10-25 Plug / Plug Back No Violations I was on location with DOGRM inspector, John Fleming and Columbia Gas Transmission, LLC to plug this well. Steve Hagen Well Service was the plugging and cementing contractor for Columbia Gas Transmission, LLC. The total depth of the well was at 852 feet and the bottom of the 7 inch production casing was at 824 feet. A cement retainer was set at 806 feet, leaving 28 foot of open hole below the bottom of the 7 inch casing. Hagen rigged up to set the Berea sandstone plug using 2 1/16 inch I. J. hydril tubing. Prior to setting the Berea plug, circulation was established with 15 BBL of fresh water. Hagen set the 2 1/16 inch tubing through the cement retainer to cement the open hole with 12 sacks of Class A cement. Hagen then pulled out of the cement retainer and the tubing was set near the bottom of the 7 inch casing or 849 feet. A 400 foot plug was set from 824 feet to 424 feet inside the 7 inch casing, using 77 sacks of Class A cement. Cement was scaled and verified at 14.8 ppg. Hagen pulled 2 joints of 2 1/16 inch I. J.tubing to set the top plug from 350 feet to surface inside the 7 inch casing, with 90 sacks of Class A cement. Cement was weighed and verified at 15.0 ppg. Hagen pulled the last 11 joints of 2 1/16 inch I. J. tubing from the casing and topped off the casing with any remaining cement.
0557855312 2013-05-30 Storage Well No Violations I inspected Columbia Gas Transmission gas well # 4349 today. The well is located in Section 33, Fairfield Township, Columbiana County, Ohio. The well is equipped with a master gate valve that is plumbed up to a side gate valve. The side gate valve is plumbed into a 4 lead line that goes into the main line. The annulus valve is plumbed up and is closed. There is no tank on location. The well is properly identified with a clear, legible identification sign. The lease road is in stable condition. No violations found.
0581710412 2016-05-03 Storage Well No Violations I conducted a status check on this well. The well is equipped with a master gate valve that is plumbed up to a side gate valve. The side gate valve is plumbed into a 4.00 lead line that runs into the main line. The well is properly identified with a clear, legibly marked well identification sign. The lease road is in stable condition and drivable. No violations found.
0702244765 2018-05-09 Storage Well No Violations The purpose of my inspection was to conduct a status check on well #2-0453. Access to the well remained stable and drivable. Correct identification was present at the well head. The production casing was equipped with a flow tee that contained an observation valve on the top and a wing valve on the side. The wing valve connected to a buried drip. No leaks were witnessed. No violations were observed during this inspection.
0948127829 2017-03-06 Storage Well No Violations The purpose of my inspection was to conduct a status check on well # 2-0453. Lease access was over an agricultural road that was stable and drivable. The well was located within an agricultural field stabilized with well-established vegetation. Correct and legible identification was present at the well. The well head was equipped with a 7 X 4.5swage and a production tee. The top of the tee was fitted with a closed gate valve. The side of the tee was fitted with a side gate valve in the open position. A 4 line was plumbed out of the side gate valve to a buried drip. No audible or visual leaks were detected. No violations were detected during this inspection.
0965654986 2018-11-13 Plug / Plug Back No Violations The purpose of my inspection was to conduct a check on the restoration status of well #2-0453. Access to the well remained stable and drivable. Correct identification remained in place. The location was within an agricultural field that contained established crop vegetation around the location. The crushed stone access road remained in place. An orange safety fence perimeter was present around the well head. The well head area was excavated to remove the concrete cellar and cut off the well head below grade. The lowering of the well casing had not taken place at the time of inspection. The excavated area contained meteoric water from runoff. I did not observe any sheen or gas leakage through the standing water. Filter sock was installed down grade from the disturbed areas. Equipment on location consisted of one (1) sealed roll off box and one (1) water tank. I will contact Adam Knapp with TransCanada to determine the schedule of restoration activities. No violations were observed during this inspection.
