0015688887 2019-04-11 Storage Well No Violations I was on location to check this storage well with Columbia Gas and US Department of Transportation personnel during a PHMSA (Pipeline and Hazardous Materials Safety Administration) audit of the Brinker field. The well was equipped with a master valve and a side wing valve. The annular pressure was gauged and read 156 psi The tubing pressure was 157 psi. Proper identification was posted. The access road was stable. I observed no violations at the time of inspection.

The well is equipped with a master gate valve that is plumbed up to a side gate valve. The side gate valve is plumbed into a 4 lead line that goes into the main line. The annulus valve is plumbed up and is closed. There is no tank on location. The well is properly identified with a clear, legible identification sign. The lease road is in stable condition. No violations found.

1842386510 2017-03-02 Storage Well No Violations The purpose of my inspection was to conduct a status check on well # 2-0406. Access to the well over a crushed gravel road that was in good repair. The well location was stable with well established vegetation. Correct and legible identification was present at the well. Well head annular side ports contained closed and plugged valves. The top of the well head was equipped with a master gate valve and side gate valve. A 4 line was plumbed out of the side gate valve into a vertical drip. A 4 line was plumbed out of the drip to the main line. Test valves were closed and plugged. No audible or visual leaks were detected. No violations were detected during this inspection.
1984557940 2018-02-26 Storage Well No Violations The purpose of my inspection was to conduct a status check on well #20406. Access to the well was stable and drivable. Correct identification was posted at the well. The surface casing was plumbed above grade. Annular valves were closed to atmosphere. The production casing was equipped with a master gate valve and wing valve. A 4.5 line connected the wing valve to a vertical drip. No leaks were observed. No violations were observed during this inspection.
