0525631963 2017-07-11 Plug / Plug Back No Violations The purpose of my inspection was to witness the setting of the bottom plug in well #2-0395. Well owner Bob Foltz was the plugging contractor. Mr. Foltz determined the total depth to be 840 feet GL. The static water level in the well was 150 GL. A tremie line consisting of 2 tubing was inserted into the well to a depth of 824. A total of 3.75 yards of 9 sack grout was poured into the well. The tremie line flowed during pouring of the grout. While the grout was being poured, Mr. Foltz periodically picked up on the tremie line to prevent bridging. The tubing was removed when flow stopped. The pit dug to contain the fluid returns remained stable with freeboard. No other spills or releases were observed. No violations were observed during this inspection.
0648000543 2017-06-30 Plug / Plug Back No Violations The purpose of my inspection was to conduct a status check on plugging operations on well #2-0395. Owner Bob Foltz had cleaned the well bore out to the surface casing seat. He planned to change over the bit size to make a clean out run to total depth. The location remained stable. The pit maintained adequate freeboard. No violations were observed during this inspection.
0656438054 2017-05-24 Plug / Plug Back No Violations The purpose of my inspection was to document the status of plugging operations on well #2-0395. I received a phone call from Bob Foltz reporting that he had encountered cement inside the 4.875 casing at a depth of 313 below surface. After discussing the situation with ODNR-DOGRM Inspectors Robert Roush and John Fleming, in addition to Northeast Regional Supervisor Thomas Hill and District Supervisor Stephen Ochs, the decision was made to direct Mr. Foltz to attempt to remove the casing from the well bore due to lack of zonal isolation of the productive formation. Supervisor Ochs later arrived on location to also witness casing removal. Mr. Foltz removed a total amount of 597.15 feet of 4.875 casing. A 9 sack grout plug was encountered inside the casing. Below the plug the casing remained open. After removing the casing an obstruction was encountered at 285 below surface. Mr. Foltz unsuccessfully attempted to push through the obstruction with a set of jars. Mr. Foltz stated he would return with a different tool to get through the obstruction and continue with plugging operations. I will continue to communicate with Mr. Foltz on the status of operations in order to be present during plugging operations.
0930026097 2018-06-12 Final Restoration No Violations The purpose of my inspection was to determine the status of final restoration on well #2-0395. At the time of my inspection, the working pit was backfilled and the location graded to local contour. Vegetation was established. No signs of erosion or sedimentation were observed. All equipment and debris utilized in plug and abandonment was removed. The access road was reclaimed. The location has passed the requirements for final restoration.
1449149794 2015-11-18 Production Wells No Violations My inspection found the well to be equipped with 8 1/4 casing, 6 5/8 casing, and 5 3/16 casing. There is 2 tubing in the well. The tubing is bent above the 5 3/16 packing. The annulus is open to the atmosphere. The tank battery has been removed by the well owner. There is no known production ever recorded for the well. The well owner is currently working to plug other berea wells on the property.
1469683014 2017-06-14 Plug / Plug Back No Violations The purpose of my inspection was to witness plugging operations on well #2-0395. Owner Bob Foltz encountered a bridged obstruction at 300 feet below grade. Mr. Foltz utilized a spudder rig to open the well bore up and a bailer to remove the debris down to a depth of 580 feet. The water removed from the well bore was tested with a low range Quantab. The sample result was 141 ppm chloride. Mr. Foltz planned to continue working to open up the well bore to plug the well back with 9 sack grout. No violations were observed during this inspection.
1496409705 2017-06-01 Plug / Plug Back No Violations The purpose of my inspection was to conduct a status check on plugging operations on well #2-0395. The service rig was still over the well. A downhole tool was hung inside the top of the well. The well was open to atmosphere. No fluid or gas was flowing from the well. The lined pit contained fluid with adequate freeboard. I did not observe any erosion or sedimentation issues. I spoke with Bob Foltz on 5.30.2017 at 13:15. Mr. Foltz stated that he was still repairing his other rig to move on to the well and clear the downhole obstruction. No violations were observed during this inspection.
1613221255 2017-06-15 Plug / Plug Back No Violations The purpose of my inspection was to witness plugging operations on well #2-0395. Owner Bob Foltz was conducting rig repairs. The well bore remained open down to 600 feet below grade. Static water level remained at 225. No gas was observed coming from the well. The pit contained fluid with adequate free board. No violations were observed during this inspection.
1817681489 2017-05-23 Plug / Plug Back No Violations The purpose of my inspection was to witness the setting of the bottom plug in well #2-0395. The fluid level was tagged at 225 feet below surface prior to placing tubing into the well. Bob Foltz stated he had placed 2 OD tubing into the well to a depth of 840 feet to act as a tremie tube. R.W. Sidley delivered 1.25 yards of 9 sack grout. The grout was mixed to a 5 inch slump and poured into the annular space between the tubing and 4.875 casing. The tremie tube circulated water during the pouring of grout. After the grout was poured, the tremie tube continued to circulate water to the pit before going static. Mr. Foltz then removed the tubing. I witnessed the first seven joints of tubing pull wet. Tubing lengths averaged 32 feet. The remaining tubing pulled dry. I will be on location on 5/24/17 to witness Mr. Foltz attempt to pull casing and set the top plug with 9 sack grout.
