0032812657 2017-07-24 Production Wells Violations Noted The purpose of my inspection was to conduct a status check on well #6--DOGRM. The well was equipped with tubing and rods. The pumping unit was removed. No audible or visual leaks were identified. I confirmed the location of the tank battery with Owner Shawn Watson. The tank battery consisted of four (4) steel production tanks. The tanks were empty. The tanks remained in good repair. I did not observe any holes or leaks. Load out valves were closed. Hatch lids were closed. Correct and legible identification shall be posted at the well head and tank battery within 14 days in accordance with 1501:9-9-05 (A)(10). The well was incapable of production. Under 1509.62 (A)(1) the owner of a well that has no reported production for two consecutive reporting periods as reported in accordance with section 1509.11 of the Revised Code shall plug the well in accordance with section 1509.12. Shawn Watson shall submit a written plugging schedule to Inspector Ken Frye at the Ohio Department of Natural Resources, Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685, within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. Shawn Watson will be notified if the plugging schedule is approved or denied.
0132341133 2017-10-23 Plug / Plug Back No Violations The purpose of my inspection was to witness plug and abandonment activities on well #2-0392. Owner Shawn Watson had installed 2 tubing in the well to act as a tremie line. Mr. Watson reported the static water level at 6 below surface. R.W. Sidley delivered 1.5 yards of 9 sack grout. The grout was poured into the annular space of the tubing and the fluid in the well displaced into tanks Mr. Watson had on location. After the tremie line stopped flowing, Mr. Watson removed the tubing and prepared to pull the 5 production/intermediate casing. Mr. Watson installed two (2) 15 ton jacks underneath the well head to assist the rig in pulling the casing. The initial attempt to pull the casing failed due to failure of a piece of steel on the sheave block. Further attempts to remove casing were postponed due to inclement weather. I will schedule to witness the second attempt to remove the casing. No violations were observed during this inspection.
0209454966 2017-10-16 Plug / Plug Back No Violations The purpose of my inspection was to witness plug and abandonment activities on well #2-0392. The owner was the plugging contractor. Mr. Watson was not on location during the inspection. Equipment was setup to collect fluid from the well in preparation for plugging. I did not observe any evidence of leaks or spills. No flow was observed from the well. I spoke with Mr. Watson on 10/11/2017 concerning the status of plug and abandonment activities. Mr. Watson stated that health issues had prevented work on the well. He continued that he planned to resume activities the week of 10/16/2017.
0288628961 2017-11-08 Final Restoration No Violations The purpose of my inspection was to document the status of final restoration on well #2-0392. I met owner Shawn Watson on location. Mr. Watson was still in the process of transferring recovered well fluid and removing equipment associated with the plugging operation. Final restoration was not passed at this time.
0340101085 2017-11-29 Plug / Plug Back No Violations The purpose of my inspection was to witness the owner Shawn Watson cut off the conductor casing and weld the monument plate onto well #2-0292. Mr. Watson cut the conductor casing at 36 below surface grade and welded a monument plate onto the casing. Material recovered from plugging operations remained on location. Mr. Watson stated that he planned to remove the tubing that day. I will schedule to re-inspect the location to determine final restoration after the material is removed.

Operator Shawn Watson had removed the rods from the well. The location remained stable. No pits were dug. While on location I reviewed the records for water well #875169 and #610943. Well record #610943 reported 300 mg/L of sodium chloride during the pumping test conducted on April 1983. I will schedule to be on location during all plugging activies.
0776747173 2017-10-18 Plug / Plug Back No Violations The purpose of my inspection was to conduct a status check on plug and abandonment activities on well #2-0392. Owner Shawn Watson had ended activities for the day. I observed the 0.75 gas relief tubing removed from the well and placed on the ground. Mr. Watson had begun to remove the 2 production tubing. The location remained stable. No flow was observed from the well. I will schedule to be present for future plug and abandonment actives.
0794520965 2017-10-31 Plug / Plug Back No Violations The purpose of my inspection was to follow up on compliance notice #32812657 . The surface plug was set on 10/27/2017. Owner Shawn Watson reported pouring 9 sack grout to surface. At the time of my inspection the top of the cement plug was at 13 feet below surface. Mr. Watson arrived on location during the inspection and reported that he would fill the well to at least 36 inches below surface with bag cement mix prior to cutting off the conductor casing and welding on a monument plate. Well casing and tubing remained on location. I will schedule with Mr. Watson to conduct an inspection for final restoration once all equipment is removed.

The well access and location remained stable. Owner Shawn Watson had ended activity for the day. I observed the 1 gas relief tubing on the ground. Mr. Watson had begun to remove the 2 production tubing from the well. I did not observe any spills to the ground. The well was open and not flowing any gas. I will schedule with Mr. Watson to be on location to witness further activity. No violations were observed during this inspection.

1509456404 2017-10-27 Plug / Plug Back No Violations The purpose of my inspection was to witness plug and abandonment activities on well #2-0392. Owner Shawn Watson reported the well bore was open to 479 feet below surface. A total of 237 feet of 6 inch casing was recovered . The static fluid level in the well was 5 feet below surface. A tremie tube of 2 inch tubing was installed in the well to a depth of 460 feet. R.W. Sidley delivered 4.0 yards of 9 sack grout. The grout was poured into the annular space of the tubing. When the tremie line stopped flowing, Mr. Watson started to remove the tubing from the well. Mr. Watson continued to remove tubing at the conclusion of my inspection. Approximately 0.5 yards remained on the truck to top off the well after the tremie line was removed. No violations were observed during this inspection.

1731618049 2017-10-19 Production Wells No Violations The purpose of my inspection was to witness plug and abandonment activities on well #2-0392. Owner Shawn Watson was on location removing the 2 tubing from the well. Mr. Watson reported in a phone conversation earlier in the day that the tubing had parted and he needed to utilize a fishing tool to recover the remaining tubing. Mr. Watson recovered all of the tubing from the well and needed to additional tubing to reach the tremie line depth due to the worn condition of some of the tubing. I will schedule to be on location to witness the placement of the zonal isolation plug.
2071670334 2017-10-26 Plug / Plug Back No Violations The purpose of my inspection was to witness plug and abandonment activities on well #2-0392. Owner Shawn Watson reported the top of the first plug was measured at 485 below surface. No flow was observed from the well. I witnessed Mr. Watson make a second attempt to pull the intermediate/ production casing after installing a new pulling block. The second attempt was unsuccessful. Mr. Watson unscrewed the casing and began to remove the casing from the well. I witnessed 180 feet of casing removed from the well at the conclusion of my inspection. Additional casing remained in the well and was being pulled. I will schedule to be present on 10/27/2017 to witness the setting of the surface plug.
