0543501240 2018-06-27 Storage Well No Violations The purpose of my inspection was to document the status of well #2-0186. At the time of my inspection the well head leak was not repaired. On 6/29/2018 I spoke with Jessica Carothers with Lakeshore Gas Storage. Ms. Carothers reported that the leak was reported and a repair order was issued for the well. She did not know when repairs would be made. I requested to be notified when repairs were to take place in order to schedule to be present.
0549339275 2018-08-23 Storage Well No Violations The purpose of my inspection was to conduct a status check on well #2-0186. ODNR-DOGRM Northeast Regional Supervisor Thomas Hill was present during the inspection. Access to the well head remained stable. The posted identification sign indicated the incorrect operator, Lakeshore Gas Storage. The well head was below grade. The injection casing was equipped with a master gate valve topped with a flow cross. The top of the cross contained a snubbing head and single pin chart. The sides of the cross contained wing valves. The West wing valve was capped and the East valve was connected to a 4.5 line. All valves were closed. The gas leak witnessed during the inspection on 06/27/2018 (Inspection # 543501240) remained. On 9/5/2018 I spoke with Jessica Carothers of Lakeshore Gas Storage to determine the status of the repair order. Ms. Carothers stated that Lakeshore Gas Storage that ownership of the field was transferred to U.S. trustee Attorney Tony Degirolamo. Ms. Carothers continued that Mr. Degirolamo was currently responsible for the field and would be the point of contact for any maintenance or compliance issues. Additional investigation regarding well ownership is required.
0551542380 2013-03-07 Storage Well No Violations I inspected Dominion East Ohio gas well # 1974 today. The well is located in Section 2, Knox Township, Columbiana County, Ohio. The well is equipped with a master gate valve, a side gate valve with a 4 daniels run plumbed into the main line. There is a flow gate valve with a seal plate on the end. There is no tank on location. The well is properly identified with a clear, legible identification sign mounted on the main line. The wellhead is leaking at the ground around the collar. Notified Steve Lawlis with Lake Shore Gas Storage and he said he would have it fixed. The lease road is in stable condition.
0774618044 2018-06-27 Storage Well No Violations The purpose of my inspection was to document the status of the gas leak observed at the well head. At the time of my inspection the well head continued to leak gas from the below the water in the concrete vault box. Following the inspection I spoke with Lakeshore Gas Storage representative Jessica Carothers. Ms. Carothers stated that she had reported the condition of the well and a repair order had been issued for well #2-0186. She did not have a scheduled date for repairs. I requested to Ms. Carothers notification of when repairs were to be made and she agreed. I will schedule a follow-up inspection to document the status of repairs once the work is completed.
1203608930 2014-08-27 Storage Well No Violations I was on site to conduct a status check. Gas was bubbling up from around the base of the wellhead. I contacted Lake Shore Gas Storage and told them about the leak, and they said they would get it fixed and give me a call when they did.

There was a sales line with a separator. There is no tank associated with this well. The well is properly identified with a clear, legibly marked well identification sign. The well was equipped with a master gate valve, side gate valve, a 4.5 flow line with flow gate valve and a domestic supply connection. No leaks were witnessed. Ownership and the well name need to be updated.
1678196619 2018-06-07 Storage Well No Violations The purpose of my inspection was to conduct a status check on well #2-0186. Access to the well remained drivable and stable. Identification at the well head indicated Lake Shore Gas Storage as the owner. A concrete perimeter was present around the well head and holding meteoric water. I observed gas bubbling up through the water. Opaque conditions and depth of the water prevented making an accurate determination of the location of the leak. The injection casing was equipped with a master gate valve that was topped with a flow cross. The top of the cross contained a single pin chart. The sides of the cross contained wing valves, with the West valve being capped and the East valve being connected to a 4.5 run. All valves were closed. No other leaks were observed. A domestic supply connection was present. While on location I called and spoke with Jessica Carothers at Lakeshore Gas Storage to notify the operator of the leak. Ms. Carothers stated she would have someone out to investigate the leak and make necessary repairs as soon as possible. I will schedule a follow-up inspection to document the status of repairs.

There is a flow gate valve plumbed with a 3.00 line into a 100 BBL storage tank. All lids and hatches on the storage tank are closed and secure. There are two (2) tank drains on the storage tank and they are secured with valves and bull plugs in place. The tank battery is diked. The well is not properly identified with a clear, legibly marked well identification sign mounted on the storage tank. The lease road is stable and drivable. Well signage needs updated.
