0079548310 | 2014-07-20 | Casing String Witnessed | No Violations | I witnessed the cementing of the conductor casing. TopHole Drilling drilled a 26 diameter hole to 120. TopHole installed 120 of 20 OD casing. O-Tex Pumping was the cementing contractor. The cement was mudscaled, and O-Tex pumped 53.4 bbls of cement slurry at 15.6 pounds per gallon. The cement was displaced with 39 bbls of water. 19 bbls of cement returned to surface. Displacement was completed at 5:15pm. |
0162995461 | 2014-11-12 | Drill / Deepen / Reopen | No Violations | I was on site to check the condition of the location following drilling operations. All drilling equipment has been removed including the containment layer and rig mats. There is a forklift, welding equipment, and steel flow line on location. All seven wellheads are secure and the tin horns and wellheads are gated with steel fencing. No violations. |

0837303652 | 2014-10-29 | Casing String Witnessed | No Violations | I was on location at 5:30pm to witness the cementing of the 5.5 production casing. Nomac Rig #73 drilled a 8.5 hole to a depth of 14,474 and Wyoming Casing landed the production casing at the bottom of the hole. O-Tex rigged up to the Blackhawk rotating tool and cemented the casing with 280 barrels of lead cement and 337 barrels of tail cement. They dropped the plug and displaced the plug to the collar with 319.4 barrels of water. The plug landed at 2100psi and the pressure was increased and held at 2600psi for five minutes. 19 barrels of Barite spacer circulated to surface. See cement ticket for more details on this job. No violations. |
1021121517 | 2016-03-01 | Production Wells | No Violations | I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road gate was in good repair, but was unlocked at the time of inspection. The #8H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and equipped with locks on the man door. The man door was open at the time of inspection, but Chesapeake personnel were on site. The equipment on the pad consists of the following:
9 - 400 bbl production tanks, three labelled as containing produced water, six labelled as containing condensate. 1- Low-pressure vertical separator located inside of the dike of the tank battery. 7 Producing wells 7 Sand separators 7 Gas Processing Units 5 Line Heaters 1 Heat Treater 1 Vapor Recovery Unit 2 Bullet Tanks |
1088697310 | 2014-10-22 | Casing String Witnessed | No Violations | I was on location at 3:15pm to witness the cementing of the 9.625 intermediate casing. Nomac Rig #73 drilled a 12.25 hole to a total depth of 2410 and Wyoming Casing set 2400 of intermediate casing in the hole. O-Tex Pumping rigged up, tested the lines, and cemented the casing with 185 barrels of Class H cement slurry. They released the plug and displaced the plug to the float collar with 177 barrels of freshwater. The plug landed at 1100psi and the pressure was increased and held at 1630psi for 5 minutes. The pressure was released and 1.0 barrels flowed back to the truck. 36 barrels of cement circulated to surface. No violations. See cement ticket for more details on this job. |
1424179825 | 2014-10-21 | Casing String Witnessed | No Violations | I was on location at 4:50am to witness the cementing of the 13.375 surface casing. Nomac Rig #73 drilled a 17.5 hole to a total depth of 860 and Wyoming Casing set 850 of surface casing in the hole. O-Tex Pumping rigged up, tested the lines, and cemented the casing with 165 barrels of Class H cement slurry. They released the plug and displaced the plug to the float collar with 125 barrels of freshwater. The plug landed at 350psi and the pressure was increased and held at 850psi for 5 minutes. The pressure was released and 0.5 barrels flowed back to the truck. 44 barrels of cement circulated to surface. No violations. See cement ticket for more details on this job. |
1600703384 | 2014-10-31 | Drill / Deepen / Reopen | No Violations | I was on location to check on rigging down operations of Nomac #73. The derrick has been laid over and trucks are being loaded. I picked up the 5.5 production casing cement tickets from the job on 10/29/14. Loads will be hauled to the Mattie Pad in Harrison County beginning in the morning. No violations. |
1752369451 | 2015-10-08 | Preliminary Restoration | No Violations | I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of nine, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:
Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed. |
