0003291819 2016-03-01 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road gate was in good repair, but was unlocked at the time of inspection. The #1H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and equipped with locks on the man door. The man door was open at the time of inspection, but Chesapeake personnel were on site. The equipment on the pad consists of the following:

9 - 400 bbl production tanks, three labelled as containing produced water, six labelled as containing condensate. 1- Low-pressure vertical separator located inside of the dike of the tank battery. 7 Producing wells 7 Sand separators 7 Gas Processing Units 5 Line Heaters 1 Heat Treater 1 Vapor Recovery Unit 2 Bullet Tanks

0095389863 2014-08-10 Casing String Witnessed No Violations I was on location to witness the cementing of the 9.625 surface casing. Nomac #73 drilled a 12.25 hole to a depth of 2,010 and Wyoming Casing set the pipe at a depth of 1,999. O-Tex Pumping cemented the casing with 710 sacks of Class H cement weighing 15.8lbs/gal on the pressurized scales. 25 barrels of cement slurry circulated to surface. See cement report for more details on this job. No violations.

Nomac #73 drilled a 17.5 hole to a depth of 860 and Wyoming Casing set the pipe at a depth of 850. O-Tex Pumping cemented the casing with 770 sacks of Class H cement weighing 15.6lbs/gal on the pressurized scales. 45 barrels of cement slurry circulated to surface. 0657967386 2014-07-28 Drill / Deepen / Reopen No Violations I was on location to inspect the status of the well pad following the cementing of conductor casing last week. Contractors were on site grading and compacting the location in preparation of Nomac Rig #73 moving in. No violations.
1029166381 2015-10-08 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of nine, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1482329809 2014-08-13 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #73. They drilled the 8.75 section to the mud-up-point of 7,634. They are currently drilling the 8.5 curve at a depth of 7,712 with synthetic, oil based mud. The location is in good condition and all equipment is on containment. No violations.
