0600381720 2015-10-08 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of nine, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0634313588 2014-08-29 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #73. They are currently drilling the lateral at a total measured depth on 9,910 with synthetic, oil based mud (SOBM). The kick-off point was at a depth of 7,640 and the horizontal leg is being drilled at a vertical depth of 8,698. The location is in good condition and all drilling equipment is on location. No violations.
0715243169 2014-07-20 Casing String Witnessed No Violations I witnessed the cementing of the conductor casing. TopHole Drilling drilled a 26 diameter hole to 120. TopHole installed 120 of 20 OD casing. O-Tex Pumping was the cementing contractor. The cement was mudscaled, and O-Tex pumped 53.4 bbls of cement slurry at 15.6 pounds per gallon. The cement was displaced with 39 bbls of water. 19 bbls of cement returned to surface. Displacement was completed at 6:00pm.
1119227376 2016-03-01 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road gate was in good repair, but was unlocked at the time of inspection. The #2H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and equipped with locks on the man door. The man door was open at the time of inspection, but Chesapeake personnel were on site. The equipment on the pad consists of the following:

9 - 400 bbl production tanks, three labelled as containing produced water, six labelled as containing condensate. 1- Low-pressure vertical separator located inside of the dike of the tank battery. 7 Producing wells 7 Sand separators 7 Gas Processing Units 5 Line Heaters 1 Heat Treater 1 Vapor Recovery Unit 2 Bullet Tanks

1143199857 2014-08-23 Casing String Witnessed No Violations I was on location at 4:45am to witness the cementing of the 13.375 surface casing. Nomac drilled a 17.5 surface hole to a depth of 860 and 54.5lb/ft. surface casing was set at 850. O-Tex Pumping cemented the casing with 164 barrels of Class H cement and 34 barrels of cement weighing 15.6lbs/gal circulated to the cellar. See casing report for more details on this job. No violations.
1418321030 2014-08-22 Drill / Deepen / Reopen No Violations I was on location to check the status of Nomac Rig #73. They have centered the rig over the 20 conductor casing and should spud the 17.5 surface around 2:00pm this afternoon. The location is in good condition and all equipment is on containment. No violations.
