0344780114 2014-07-23 Casing String Witnessed No Violations I was on location to witness the cementing of the 20 conductor casing. The joints are butt welded and the casing is set open-ended on the bottom of a 26 hole that is 120 in depth. Holes have been cut in the casing 5-6 off bottom to allow fluid circulation. O-Tex Pumping cemented the casing with 47 barrels of 15.6lb/gal, Class H slurry (had to switch pump trucks just after starting cement). 20 barrels of cement slurry circulated to the surface. See cement report for more details on this job. No violations.
1164510470 2015-10-08 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of nine, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1422454181 2014-09-16 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #73. They are currently drilling the 8.75 vertical section of the well at a measured depth of 4570 at a rate of 200ft/hr. The location is in good condition and all equipment is on containment. No violations.
1434549814 2015-06-11 Production Wells No Violations I went to the Kennedy Pad to follow-up on complaint #4830 on 06/11/2015. The wells have been drilled and the hydraulic fracturing jobs completed. There were electricians on site during my inspection, and I was informed that flow back was to be completed in the future. I phoned the complainant on 6/11/2015 and 06/18/2015 and left messages. The complainant has not returned any of my calls. Since the complaint was in regard to excessive noise during drilling operations, and that drilling and fracturing operations are complete, the complaint can be closed.
1763735506 2014-09-23 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac #73. They are currently drilling the 8.5 lateral section at a measured depth of 15,291. Expected total depth is 15,952. I inspected the 5.5 production casing on the racks and it is new, P-110, R-3 casing. The site is in good condition and all equipment is on containment. No violations.
1953654462 2016-03-01 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road gate was in good repair, but was unlocked at the time of inspection. The #4H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and equipped with locks on the man door. The man door was open at the time of inspection, but Chesapeake personnel were on site. The equipment on the pad consists of the following:

9 -400 bbl production tanks, three labelled as containing produced water, six labelled as containing condensate. 1- Low-pressure vertical separator located inside of the dike of the tank battery. 7 Producing wells 7 Sand separators 7 Gas Processing Units 5 Line Heaters 1 Heat Treater 1 Vapor Recovery Unit 2 Bullet Tanks
